(CPS), which involved the steps like the development of the survey
along with pilot testing, implementation of the survey and finally

average number of the adults with type 2 diabetes in a community was
164. There are very limited healthcare delivery systems available to

for diabetes management. There were various funding models,
healthcare services and innovative strategies in different communities
for providing an improved care to the diabetic patients (Tompkins et
al., 2018).
