Subject: Application for reviewing reference number SY356131D of Attendance Allowance
Respected Sir,
This is in regard with the reference number SY356131D where I have been denied for the
Attendance Allowance. I hereby would like you to reconsider this decision as I am entitled for
the aforesaid allowance. This is to bring in your kind notice that I am 66 years old man who
moved to London last year on 1st May 2018 with a residence permit of 5 years. I live here with
my wife and a son. Under the Personal Independence Payment (PIP), I am an eligible candidate
due to my difficulties in mobility and doing daily chores like eating food, cleaning up, washing,
dressing and walking.
Before elaborating on my present conditions, I would like to give you a brief introduction
about my disability. In 1999, I had a stroke and it affected the entire left side of my body. It
impacted my immunity levels and hence I am still weak. Along with this, I was on strict dosage
of medicines on daily basis. This worsened my conditions in such a way that I became totally
dependent on my wife for doing all the small activities of daily routine. Thus, I retired from my
job and started living on the disability pension from my home country. I have been registered as
a disabled pensioner in my country and have been getting help and assistance from my wife for
24*7.
I also get epilepsy attacks due to inconsistencies in my medications and other related
treatments, which also affected my other organs. Furthermore, most of the times, I have high
blood pressure. Such conditions are constant and force me for an additional assistance all the
times. Hereby when my wife moved to London to my son in the year 2015 in context to
submission of all the legal documents for my smooth move. During that time, I was under the
supervision of one of my close relatives who looked after me and helped me with my daily
activities. Therefore, I would request your attention towards my prolonged conditions that is in

need of assistance for 24 hours as I might not be able to prevent any mis-happenings or accidents
to myself.
In regard to the same, I would like to draw your kind attention to understand my
conditions that have been persistent since two decades. Presently, I have settled down in London
under this country's legislative framework and have already registered with one of the General
Practitioners. I am in need of regular check-ups, treatments and medications which help me to
live my life in a controlled environment. Such consultations give me perspectives and help me to
make myself better. Along with this, this has made me understand how to respond to any
changes in relation to my disability. For instance, I am having great difficulty in walking and it is
leading to worse direction. Sometimes, such changes leave me with helplessness and frustration.
Thereafter, in this year, I am going to consult a Neurologist and a psychiatrist as well in the
month of July to get help with my disabled conditions. My brain scanning will be done and
counselling sessions might be conducted to bring improvements for my continued survival. So, it
is important for me to claim this Attendance Allowance in order to meet my expenses and
additional expenditures.
I would request you to kindly look into this decision again and review it accordingly and
help me and my family. I am in need of this allowance as I have been in constant pain and
suffering and want suitable services to continue my journey. I would be waiting for your reply.
