Description
In week 5, 6 and 7, the focus was on assessment of patients through the application of
Roper, Logan and Tierney model of nursing to assess patients and determine their ability to cope
with clinical situations. The Roper, Logan, and Tierney model of nursing is based on activities of
daily living that is used to assess how the life of patients changes due to illness, injury or
admission to a healthcare facility (Williams, 2015). This means that the focus of the theory is to
understand the relative independence and potential for independence in daily life. It has to be
noted that patient-centered approaches are based on determining the needs of the patient in the
areas of communication, berthing, eating and drinking, washing and dressing, elimination,
mobilization working and playing and sometimes sexuality activities of daily living that depend
on the situation of the patient. The role of this theory in nursing care is to develop appropriate
competencies for meeting the needs of the patient.
When applying the theory and patient-centered approaches, it is important to note that
every patient presents unique situations that require critical analysis to develop a care plan that
will address the deficiencies that the patients have (Hill, Hall & Glew, 2017). As a professional, I
need to understand the application of patient-centered approaches in the clinical environment and
have ways of ensuring that I am able to identify the needs of the patient and prioritize in every
way possible. This assessment can be based on the application of the clinical reasoning cycle as a
way of determining the state of the patient and determining the individual needs that need to be
done on them. Through the cycle, factors influencing activities of living like the biological,
psychological and physical are determined to understand the individual patient situations that
exist.

Feelings
Assessment of patients using the Roper, Logan and Tierney model of nursing is important
in determining the best care that the patient requires. Through the clinical reasoning cycle, I am
able to use the eight steps of assessment to determine the state of the patient and the ability to
deal with the states mentioned in the description step. This means that the ability to consider
facts from the patient's situation, collecting the information, processing it, identifying key
problem areas, establishing goals, taking action and developing ways of evaluation are important
in patient-centered care.
When dealing with patients with the centered care approach, it is important to assess the
patient and determine the requirements and challenges that each of the patient presents. I this
case the Roper, Logan, and Tierney model of nursing is important in assessing the ability of the
patient to manage personal life. In the case the patient presents different challenges, then the
action that is developed has to reflect the individual needs that the patient has (Gluyas, 2015).
Thus I have gathered adequate knowledge and skills that can be used to assess and determine the
different needs that each patient presents and ensure that I am able to understand the needs that
each of them presents. After accomplishing the three weeks of work, I feel that I understand the
role of the clinical reasoning cycle in patient assessment.
Evaluation
From the Roper, Logan and Tierney model of nursing, healthcare professionals require
clinical assessment competencies that can be used to collect data from the patient that informs
decision making. This means that clinical reasoning ability is important in the application of the
model to gather relevant information that reflects the daily needs of the patients. However, I
discovered that I did not have clinical assessment skills. In the patient assessment, information

can be gathered through observing the patient and asking of clinical question. Thus for clinical
questioning to be effective, then I need to have proper skills that can be used to gather relevant
information from the patient. According to Ament, et al. (2018) the professional uses
communication skills to establish rapport with the patient which leads to collecting adequate data
for decision making.
Analysis
In clinical application of the application of Roper, Logan and Tierney model of nursing I
realized that communication and assessment skills are important in determining the daily
activities of the patient. Since patients come from different groups that require different
communication and assessment skills, then it is important for me as a clinical professional to
have the best clinical skills that can generate adequate knowledge (Levett-Jones, MacDonald-
Wicks & Oates, 2014).. This means that to apply different clinical and assessment skills that will
lead to gathering enough data. For example, by speaking to the patients well I am able to create
an environment that makes them comfortable and able to open up thus assisting me to gather
adequate information for decision making. However, in the course of assessing the patient and
gathering information for assessing the daily activities, I realized that I did not have adequate
communication skills and critical thinking abilities that will enhance my ability to deal with
patient issues. This means that I have to improve my communication skills and critical thinking
skills so that I can be able to assess issues in the life of the patient to meet their needs.
Conclusion
From the analysis of the skills and abilities that I have, I noticed that critical thinking and
communication skills are important in using the clinical reasoning cycle to apply the application
of Roper, Logan and Tierney model of nursing. The three weeks have filled knowledge gaps in

my clinical skills but I have noticed gaps in communication skills and critical thinking. As a
professional, I have good interpersonal skills and the ability to build rapport with patients by
creating a positive environment for their interaction. However, I have to develop an action plan
that will address the deficiencies in the two areas mentioned above.
Action plan
In my action plan, I will be focusing on two areas: communication skills and critical thinking
skills. In communication skills, the focus is on improving my communication skills through
equipping myself with relevant communication tools. This means that as a practitioner, I need to
know the appropriate communication model to apply in different patient situations. Olsson, Ung,
J., Swedberg, & Ekman (2013) add that this also entails the adequate application of patient-
centered approaches as a way of ensuring that I am able to handle patients with the unique needs
that they have. In most cases, the needs of the patient vary from individual to individual and thus
when applying the Roper, Logan and Tierney model of nursing, the basis of clinical assessment
needs to be on the individual needs of the patient rather than the general perspective that exists in
healthcare.
Critical thinking skills can be improved by learning how to appropriately apply the clinical
reasoning cycle. According to Barratt (2018), clinical reasoning is supposed to assist the
professional to understand the nature of the patients' condition before problem-solving. This
means that as a professional I have to develop critical thinking skills through working with a
mentor who will assist me in reasoning and analyzing the issues that the patient present. I have to
admit that I have developed enough theory that can be used in clinical settings and thus I need to
work closely with other experienced professionals so that I can learn how to apply different
clinical strengths inpatient situations.
