Using the case study below complete the Safeguarding Adults Initial Enquiry Form. (40 Marks)
Mr C has complex care and support needs and was being supported by a
number of agencies.
Mr C experienced a series of placement moves which had an impact on his
emotional wellbeing resulting in a deterioration of behaviour.
Mr C stopped eating and drinking which led to a decline in his physical
health. Concerns were expressed by Mr C’s care team that there may be an
underlying physical cause for him not eating and drinking and losing weight.
Mr C was admitted to the local acute hospital trust for observations and
diagnostic tests. During his stay in hospital, he did not receive the necessary
tests to rule out a physical health condition as a root cause of his physical
presentation.
Mr C was discharged from hospital 7 weeks later as it was felt that his
condition was due to behavioural and not physical causes.
Mr C was discharged to a nursing home for end of life care.

and that do not discriminate: however the abuse, if there is any abuse, cannot be minimised or downplayed. The health
care worker has to find the balance of rights, risks, power and control. It is not easy to achieve.

Mr C required complex and support need in order to maintain their health and wellbeing. It has been noticed that Me C has
stopped eating and drinking everything which may lead to decline in physical health and it also impact on his emotional
wellbeing. Recently he has been admitted to local acute hospital for some diagnostic test but it has been found that no test can
rule out his physical health condition and hence later on it reveal that his condition may be due to his behaviour and not due to
any physical cause. It has been seen that Mr C has not provided family support and his family are living in different city and
hence he have to manage his work by own which could lead to change his behaviour. He has been isolated from the society and
are not involving in any social gathering which could lead to mental trauma.

In order to ensure the safety of alleged victim, various preventive measures can be taken. Under the health and safety act, every
person must be prevented from unsafe services and crime. The person who is committing crime may have to pay for it and
government can take a legal action him. When patient were showing unpredictable or aggressive behaviour, health care
professional can take patient in an isolated room and provide services in order to manage their behaviour. When the patient is
aggressive, remove the entire thing that might produce any harm to the patient as well as other person. In case of any injury has
been seen called immediately to health care professional in order to manage them.

Adult may require mental and physical support in order to protect their health and wellbeing status. It has been seen that Mr C
mental condition was so poor and he was not able to take care of himself. Most of the time he was scolds other people and
showing unpredictable behaviour which could lead to unsafely of the people. Due to his aggressive behaviour, people are being
physically abuse and hence required physical care.

Adult who are at risk of unpredictable and aggressive nature of Mr C will provide various ways in order to safeguard enquiry. In
order to protect the health and safety of the patient as well as other people, the patient should be treated in an isolated room.
They could be provided counselling session in order to manage their behaviour and to stay calm in the given situation. This type
of patient must be engage in various activities in order to divert their mind. There are various social club who are providing
various activities in order to manage the free time of the patient.
