1. Background/Overview
As a support worker, part of your role is to facilitate community
integration and skills building. Developing the skills of people who
are disadvantaged will lead to empowerment an independence for
the person. Much of what we learn is from incidental learning, and
clients will learn from you as a support worker just through
observation. Each person’s skills building plan must be unique to
their individual needs. This assessment follows the CareShore
clients that narrate the course. Use the case studies to
demonstrate your knowledge of community participation and skills
development.

using a strengths-based approach, as well as supporting

Peter, 45 years, from a metropolitan city.
Peter had a fall from his roof and sustained paraplegia and an acquired
brain injury. Since his accident Peter has had some further difficulties.
He will go into the community on his own in his wheelchair and get lost.
He has become prone to aggressive outbursts when he gets frustrated
at not being able to do things he once did. His wife, who had been the
primary carer, felt unable to cope any more on her own and arranged for
Peter to live in a Supported Living home managed by CareShore. Peter’s
dream is to get back into the workforce in some capacity, to play
basketball as he once did, to manage his emotions, to access the
community independently and not get lost, and to ultimately live back in
his home with his wife.
a. Imagine you are Peter’s support worker, and are assisting Peter to

As a support worker of peter, I would like help him to improve the
existing skills which he carries and support him to build an ability to
demonstrate new skills.

morale of peter. I will guide him with proper plan and help him to
accomplish small goals and objectives.

The interest level of peter in doing these activities such as music
therapy which I have provide is quite low but I will encourage him to
make his own decision in choosing activities which he like to or
interested in.
e. Explain why it is important to involve Peter’s wife in your planning. (20-

It is very important to involve peter's wife in the process because, every
person is somehow connected with the loveone so for peter his love one
is his wife which is emotionally and mentally connected with the peter.
And another reason is that, peter's wife have experience in taking care

of her husband before she arranged support work or caretaker home.

Peter has agreed to have a planning meeting around his community
participation needs and to formally agree upon some goals. Peter’s wife
attended the planning meeting also and is supportive of his choices and
keen to assist where she can.
a. Prior to a formal planning meeting occurring, identify what kind of
assessment the following professionals may conduct with Peter.

psychological needs and wants, they can solve half

everyday activities.

Peter have fallen from great height which have
injured some of his vocal part, that is why is need to
have speech pathologist who will help him to
recover from diagnose of lack of proper speech and
ability to communicate.
Physiotherapist Physiotherapist will be very beneficial for peter as
they will help him to recover from injury with proper
exercise and fitness plan. And they even going to

use electrotherapy which help peter to recover in
fast way.
b. Why is it important for Peter to choose goals that not only teach him
new skills, but that also provide enjoyment? (20-50 words)
It is very important for peter to choose one goal or activities which not
only help him to learn a skill but to this will provide enjoyment as well.
These goals can be learning music because music will provide
entertainment to peter and keep him energetic throughout the day.

the table. As his support worker, you will assist Peter to plan and
achieve his goals. Think about possible ways this can happen.

make it as a skill. I would motivate peter to chose goals which will be
beneficial for him in the future. I will explain peter about the importance
of learning further new goals which help him to increase the existing
skills and allow him to develop new skills.

need to discuss any difficulties and give Peter constructive feedback
on this. Write a short script on how you would give this feedback in

I will be marking those areas of improvement and show him that he is
not performing well in the process with a respectful manner, this only
can be possible if I would first encourage him about the past success
which he have achieved in developing of skills and achievement of
certain goals and then mark the area where improvement Is needed.

I need to follow CareShore documentation for support to peter because
this allows me to be clear in the process of supporting and helping a
disable person. These documentations contain all the important
guideline which allow a person to care take such patience. This even
provide all legal documentation which will keep me safe in case of any
uncertainty. While recording the final support with the peter, the
documentation may be very beneficial and will be support me in case of
any uncertainty.
f. If at some point Peter decided he no longer wanted his wife involved
or wanted her to be informed of how things were going, would the
support worker adhere to this right to confidentiality, or would they
continue to involve his wife, as they have a responsibility to include
family as much as possible? Explain your answer. (20-50 words)
This many be difficult to answer because the involvement of wife is very
important for the treatment, but if the peter decided that he no longer
want his wife in this process than it might be required to fulfill his
demand for the betterment of his mental health. Peter might be feeling
helpless with his wife and might be hurting the feeling which need to be
removed for the betterment of peter in the process.
