INTRODUCTION
Meeting the Needs of Service Users is very essential for a social worker in order to meet
their emotional, social and practical needs. This helps in ensuring that they are able to maintain a
high quality of life. In order to care for them, social worker need to have suitable skills and
personal attributes. It also requires careful individual assessment and planning in order to meet
the needs of the person. The major responsibility of the allocated social worker is to monitor the
health and safety of Ms. A. Report will Assess Ms. A's care needs by creating beneficial and
achievable goals. For the same Multi-disciplinary approach will be applied and further it
explores the own skills in the evaluation of care needs with the help of appropriate theories.
Report will also include the models of health and social care that social worker decide to choose
in order to have quick recovery of Ms. A. Further report will include Framework for care
planning and reflexive report of own practices of social workers that will evaluate the role in
implementation of the care to meet Ms. A's needs.

The major responsibility of the allocated social worker is to monitor the health and safety
of Ms. A. Though it is extremely challenging for the social worker due constant fluctuation in
Ms A's mental state and its was directly her physical health as well as change in behaviour was

to receive long term therapeutic intervention. Although it was made clear to the social worker
that in order to receive support from CMHT, Ms. A have to willingly approach their service.
Despite this the social worker put efforts in order to encourage Ms. A to have support from
CMHT, but she refused as was having fear to be sectioned under the Mental Health Act 1983.
As the social worker met Ms. A, she represented a stable and good insight into her health
needs and was assessing as if she is having the capability to make her own decisions. She was
demonstrating independent living skills in order to assure the social worker that she is having
independent living skills in order to manage her own needs. There was many problems because
of which she was mentally ill that included frustration related to housing application because of
which rapid decline in her mental health was seen. This case for complex enough for the social
worker as Ms. A was facing homelessness and was also facing regular hospital admission due to

and have proper communication with her. Social worker gathered different information related to
health of Ms. A and also was able to fix all the problems by having responsive nature towards
Ms. A. Social worker put efforts in order to maintain the effective relation with the patient so
that she feel free to share the problems that she is facing over the years.
Application of Multi-disciplinary approach:-
For the same Social worker applied multi-Disciplinary approach in order to remain
involved and ensure that Ms A is at secured place of shelter and also to manager her care needs.
Social worker was getting regular guidelines from the manager and they have meetings in order
to explore multi-disciplinary approach in order to manage Ms A's care and support needs. They
also involve her family, members of health professional, the housing department and
occupational therapy(Tieman and et.al., 2017). This approach helped social worker in identifying
the problems that she is facing and to have guidelines that are supportive to solve those problems
in effective manner.
Exploring own skills in the evaluation of care needs
There are needs of different skills in order to evaluate the care needs by the social
worker. These skills are required to deal with the situations that Ms A is facing and these skills
are helpful in reducing the safety issues and to increase the retention rate of Ms A(Wilson,
Woollands, and Barrett, 2018). Some of these skills are as follows:-
Active listening skills:- Social worker is required to have active listening skills that is to
give time to Ms A, in order to explore her thoughts and feelings so that active measures
can be taken to overcome the issues she is facing(Kratochwil, 2017). For the same Social
worker used Constructivism theory that have focus on the differentiability the way Ms. A
make sense of different things and for the same social worker used person-centred
messages that is easily understood by Ms. A.

intentional behaviour as well as the expression of emotions that Ms. A is having.

not able to receive housing on an immediate basis and that was resulting in personality
disorder and social circumstances. Social worker did not use Empower model here as this
model emphasis on individual ability to have control on own health, but Ms. A was not
able to handle the same individually(Thomas and Milligan, 2018). So it was an effective
decision for social worker to go with social model as its helped the social worker to cope
with health condition of Ms. A.

how she interacts with her environment, rather than just focusing on illness or any

with a common formats. It is integral part of practice and include scheduling of
appointment with Ms A, check-in of Ms A, rescheduling and medical billing. This
framework helped the social worker to analyse Ms A's progress so that proper measures
can be taken to improve the same(Allen, 2019). The used framework was more useful as
compared with DAPE that is data, assessment, planning and evaluation and DAP
framework as SOAP framework it gives standard format for organising Ms. A
information and its become easy for the social worker to identify the problems that she is
facing. It helped out to maintain records by collecting the pieces of information related to
health issues she is facing and to formulate the plan of care for her.

is encouraged by the social worker to be as independent as possible. The major
assumption of this framework is that Ms. A have to be self reliant and responsible for her

framework as it is brings out good components of Ms A with respect to self care demands

April 2015, she was allocated to me for conducting its need assessment in order to assess that
whether she is eligible for service under Care Act 2015 or not.

Feelings- with the analysis of the situation of Ms A I felt that she is in a very poor
condition. But when I met her, she was presented in a stable manner and she demonstrated a
good insight into her health needs. With my analysis I felt that she was having the mental
capacity to make her own best interest decision for her and also demonstrated to have
independent living skills
Evaluation- with the analysis of the lady I evaluated that with help of application of multi-
disciplinary approach and as a result of this I was involved with Ms A and this ensured that Ms
A is at a secured place and is also able to manage her needs. Further with the evaluation I learnt
that I also require some skills to deal with the patient in proper way. These skills are like active
listening skills, conflict management, stress management and many other skills.
Analysis- with the analysis of the given situation I learnt that application of some models
relating to health and social care helped me in analysing the situation in more proper way. I
learnt that application of biomedical model helped me in analysing the biological factors of the
lady that what disease she was suffering from. With this model I learnt that there are two aspesct
in biomedical approach that is diagnosis and intervention.
Conclusion- till the end of the whole analysis of the situation of Ms A it was concluded that
she was suffering from many diseases like visual impairment, diabetes, renal failure, emotionally
unstable personality disorder and many other diseases. But with help of different models like
biomedical model of health and social care it was concluded that diagnosis of the lady included
the identification of the illness suffered by lady and intervention includes the action which I can
take to improve the situation of the lady.
Action plan- in the end for improving the condition of the lady I used two different
frameworks that is SOAP and OREM. Under SOAP framework that is Subjective, Objective,
Assessment and Plan I made notes of the condition of patient in form of chart and includes
scheduling of all the activities of Ms A. On the other hand, OREM is a framework which is used
as rehabilitation and primary care setting where I encouraged her to become self-reliant and
responsible for her own care and improving her own knowledge.
CONCLUSION
With the help of the study of the above report it is interpreted that meeting the needs of the
service user is the major requirement of a social worker. It is very important for a social worker

good insight into her health needs. With my analysis I felt that she was having the mental

to have good skills to help the service user to achieve and overcome the problem with which the
service user is going through. The report started when the social worker started assessing the
needs of the patient and to monitor the health and safety requirement of the patient.
With this the social worker analysed that Ms A is directly affected with physical health
and because of this there are changes in the behaviour of her. For this the worker applied the
theory of Multi- Disciplinary approach. This approach was applied in order to get involved with
the patient. Also, it was analysed that to deal with a patient the social worker needs to possess
some skills. These skills are like active listening skills, dealing with challenging behaviour, stress
management and many other different skills. Also, the social worker for dealing with the patient

and because of this there are changes in the behaviour of her. For this the worker applied the
theory of Multi- Disciplinary approach. This approach was applied in order to get involved with
the patient. Also, it was analysed that to deal with a patient the social worker needs to possess
some skills. These skills are like active listening skills, dealing with challenging behaviour, stress
management and many other different skills. Also, the social worker for dealing with the patient
had used different models of health and social care such as biomedical model, holistic model,
social model and many other different types of model. Further the social worker used different
framework for care planning like SOAP, OREM framework and many other different type of
