INTRODUCTION
Currently ,health and social care is very important for every person. Every person wants
to remain fit and healthy. So for it ,it is necessary for health professional that he should have all
equipment that are necessary for providing the best services to the individual and he should also
understand the requirements of the individual. In today time ,technology is upgrading and
provides the best services to the individual so that they can remain fit and healthy (Moule and
Hek, 2011). This project is based on SHARP team [Social inclusion ,Hope And Recovery Project
] that is a community based team that conducts healthy living programme and social programme
that helps the individual in remain fit. This report will explains concept of health ,illness or
disability and how perception is changing today related to the health and social care and impact
of legislation or rules on these services and currently what are the system available that full fills
the needs of the individuals.
TASK 1
1.1 Concept of illness ,disability ,health and behaviour
Health means the fitness of a individual in terms of physical fitness ,mental and social
status. It has been defined as a status of happy and comfort.
Illness – According to WHO “It means the temporary defect or temporary poor condition
of the health ,mental or social health of a person.” When a person is affected by some diseases
then it is known as illness. For example - In the given scenario ,there is a old man. Suddenly his
blood pressure becomes high and have mental illness, it is a temporary defect to the old man so it
is known as illness. Social and health care workers help the person in preventing this illness by
using their knowledge and skills.
Disability – it refers to health condition or physical impairment that prevents the
individuals in living the normal life. It has two types of disabilities which is permanent disability
or temporary disability. In permanent disability ,person can not work in in their whole life while
in temporary disability , person can work after some time (World Health Organization, 2012). So
social and health care workers should have the proper knowledge about this disability so that he
can help the affected person. For example – Mr. Benjamin have diabetes that is a disability
because in this person can not live his normal life.

All these factors are interrelated with each other in order to provide appropriate treatment
t to their. It can increases the market growth of that organisation. For every service provider, it
must have to they have to take care about clients healths, behaviour, disability and illness.
Medical model is used in psychology ,that indicates about the abnormal behaviour and
physical problems and should be treated medically. Sometimes it arises because of brain injury.
According to this model disability is a medical model by which illness or disability the result of a
physical condition intrinsic to the individual. Because of this individual life may reduce and
cause clear disadvantages to the individual. This model is also known as individual model.
According to the social model ,disability is caused by the way society is organised rather
than by a person's impairment or difference. It looks at ways of removing barriers that restrict
life choices for disabled person. It is helpful in removing the restrictions from the life of
individual who have disability. When disability has finished then disabled person becomes
independent and be able to control their life.
A professional of health care should have knowledge about all these problems so that he
can deal with these problems. Mr. Benjamin who have mental illness that will be included in the
illness because it is a temporary defect or condition of him and cerebral palsy will be included
temporary disabilities.

reduce the cost of looking after the poor as it stopped money going to poor people except in
exceptional cases. This law protects the poor from the rich men or from exploitation that are
done by rich men. This law provides all right to poor.
Disabilities Discrimination act protects the patient from discrimination that are done on
the basis of discrimination.

Mrs. Sarah is a old women that have many problems like she is partially deaf and blind
and have no proper communication skills and so on. This lady has requirement of some caring
and treatment from the professional that have knowledge and skills to solve these problems. Any
one can take services from the professional of health and social care (England and et.al., 2012) .
In this situation ,SHARP team is very helpful in providing the services to the resident of UK.
Mrs Sarah has some problems that requires some specific attention from the professional.
Health and social care workers have responsibility to clearly know about the problems and give
recommendation to her so that she can improve herself. And after that, they should create the
person centred approach that is explained in the nursing code of conduct. And next that ,all
necessary drugs or recommendation should be given to that lady.

or strategies that helps in achieving the desired result. Approaches are as follows-
ď‚· Evidence based practise - In this ,affected person receives the services from the
provider. This approach helps in providing the integrated scientific services or better
quality of services and provides the proof to the clients.
 Self -help- approach – In this approach individual with specific needs are supported to
help by themselves. For example – In SHARP team, patient is said to known , if they can
then can pick any services in their choice.
 Advocacy approach – This approach is helpful in legal cases. It helps the individual by
reporting of abusive to the police that will help in getting the justice and all equal rights.
This approaches helps the disabled person in getting the equal rights and development of
