Mathematics Attitude Impact Survey

This is a survey questionnaire designed to investigate the relationship between students' attitudes towards mathematics and their academic achievement in the subject. The questionnaire, created by Shaheeda Ali, a student at Mianz International College pursuing a Bachelor's degree in Early Childhood Education, aims to gather data for her research. The survey is divided into two parts: Part 1 collects basic demographic information (gender), while Part 2 assesses attitudes using a Likert scale. Part 2 is further categorized into sections measuring cognitive, affective, and behavioral aspects of students' attitudes, along with a section on their self-reported achievement. The survey seeks to understand how these different facets of attitude influence students' performance in mathematics.
