INTRODUCTION
Significant life events means occurrence, event positive or negative etc. after whose
occurrence life of individual does not remain same, and they are not able to go back in life which
was before the event. These events will create impact on the life of person who is suffering from
significant event such as it may lead to depression, problem in working for long hours etc. The
Report will outline the reflection of domiciliary care worker on working with patient from car
accident and other is patient suffered from dementia. The Report will also describe the ways in
which support has been given to the patients to provide caring and support.
LO 1
SET 1
Impact of significant life event on individual and family.
First case study is related with the women who have suffered various injuries due to car
accident. Client has suffered injuries to leg, arms and chest etc. Another case study is related
with 37 years old male who is suffering from gradual cognitive decline, disorientation,
disturbance in sleep etc. The disease has been started 3 years back. There are various problems
that are faced by the women due to accident such as physical challenges like minor sprains etc. It
also has emotional effects such as anxiety for a long time. Difficulties in handling different
situations etc. Further, it also impacts socio-economic status of the individual. Moreover, it also
created high level of stress (Women receives settlement for car accident, 2017).
Physical impact-
Individual and their family gets impacted due to important life event, they get scattered
and weakens from poor physical and mental health as well as suffers from many diseases.
Dementia is term for conditions and diseases characterized by decline in language, memory,
issues solving and another thinking abilities that affects individual capability to perform
everyday functions. The person who face dementia disease it impacts on their physical ability in
which they cannot be able to do daily work. It makes them able to take stress and they get
disappointed in their life. Similarly, car accident leaves a person traumatized and instils in them a
fear that they might not be able to drive the vehicle ever again.
Social impact-
Due to car accident women cannot be able to communicate with other people in society
because of injuries as they can’t walk and do their daily basic activities which affects on their

social life. The patient with dementia might become socially alienated because they are not
involved in the social life.
Emotional impact- gradual cognitive decline
The individual gets mentally and emotionally disturbed due to because it impacts on their
personal as well as professional relations.
Intellectual-
The ability to think and react is part of mental well-being which get affected due
dementia in which person totally lost their memory for some time or may be for longer.

Significant life events that are suffered or suffering by individuals create an impact on
themselves as well as on their family members as well. As a domiciliary care worker I have been
working an organization from past 2 years in UK. Positive impact is that person suffering from
significant life event receives the emotional and financial support of family. Wheeras, negative
impact is that it affects socio-economic, physical and mental condition of people. Further, when
a person suffer from disease like dementia or car accident etc. Event also affect the behaviour
and mentality of people that are surrounding the patients.
Significant life event impact individual in different stages such as psychological, social
and physical. I have experienced that, in first case study when female has suffered from car

accident the event has created negative impact on mentality and also cause physical injuries to
the women. Another common thing is emotional disturbance as patient. Significant event
changes the mentality the patient started thinking that they will not be valued by the society in
case they have suffered physical injuries. Therefore, in such a case individual become depressed,
irritable and also suffer from anxiety.
Further, it is normal that people have strong physical and emotional reaction after the
distressing event. On most of these types of occasions, reaction to the event subside as an
element of natural healing of the body and process of recovery. I have noticed that, Family
members of the patient that have experienced distressing event become closer and appreciate
each other more. Due to car accident as described in first case study, women have suffered high
level of physical distress, emotional and psychological distress that has disrupted the ability to
perform routine functions.
In my view, all the family members and friends of the person who have suffered car
accident will react in their own way. In case if family members do not understand the experience
of each other then it will lead to misunderstandings and breakdown in communication.
Moreover, family members of the person who face experienced significant life event should be
aware of the common reactions of another such as feeling extremly tired, feeling very tired,
stressed, not want to leave a particular place etc. I have seen various incidents while working as a
domiciliary care worker that family life has various changes after the person suffered the event
like car accident. Family members may feel insecured or at the thought of having so much to do.
Every person may have struggled to understand what has happened due to accident. Arguments,
misunderstandings over small things can also affect family relationships.
I have realised that, it is normal for people living in sociey to react in different ways in
respect of person suffered from single life event. The event may have positive or negative impact
on the life of person. There may be physical symptoms like headache, broken sleep etc. or it may
also affect thinking of the person who have faced the accident such as short term memory
problem, inability to absorb information etc. There may also change in the behaviour of person
like change in eating pattern, engaging in self-harming activities etc (Mole and et.al 2019).
In my opinion, family members play an important role to prevent the patient from
depression. In another case study, old man is suffering from dementia. When individual is
suffering from such type of disease in that case, it is very important that they will receive special

care and attention to their needs. Further, it is very crucial that friends and family members to
provide support to the patient who has faced significant life event like car accident or being
suffering from disease like dementia. I thought that, old age people require more attention and
support as compared to middle age group people. These people play a significant role in helping
the person to undergo such type of life events with comfort and ease. Support of family members
is important for the patient to overcome the significant events (Naveh and Katz-Navon, 2015).
In my opinion, individual who have suffered from accident requires help from family
members in performing various tasks like going to wash room, eating etc. It is very common that
people will have high level of emotional stress after going through accident as it creates anxiety,
depression, change in the behaviour etc. Other than this, such type of life event may also result in
loss of income because disability may prevent individual to return to work. Moreover, in second
case study in which old man is suffering from dementia. The disease impacts the life of person
including friends and family. Suffering from dementia impacts the individual in psychological
and emotional way. Dementia can create huge impact on the life of person. Dementia cause
disorder of brain and it happens due to failure of nerve cells. Family members have to make
various adjustments. Close family relationship is essential to help individual suffering from
dementia. I have experienced that, person suffering from dementia experiences various emotions
such as fear, grief, disbelief, anger, shock etc. During this time, patient have to struggle for
dealing with these types of emotions. They also afraid regarding their future and forgetfulness,
and they may become upset due to the impact of disease on people around them (Birks and
Harvey, 2018). The accident leaves a person both in physical as well as mental trauma and in
order to address this problem it is essential that the person is given significant life support to the
so that the advantages can be drawn in an effective manner.
Family members and friends should make efforts to maintain the level of self-esteem of
person who is suffering from dementia. They should help patient to maintain social relationship.
Further, they must avoid harsh comments. I also thought that, disease like dementia can affect
the life of family members. Physical, financial and emotional pressure of person suffering from
dementia can cause stress to family members and friends. Further, the disease can directly affect
the ability of patient to engage in social network sometimes they also decide to give up
socializing at the time they know that they have disease (Scales, 2017). Therefore, there are

various negative impact of these type of significant life events like reduced self-esteem,
decreased social engagement etc.
I thought that, the disease of dementia may cause stress for whole family members. The
impact on whole family can be overwhelming. The diagnosis of dementia may trigger various
types of emotions like sadness, anger, fear and frustration etc. Conflicts in family are common.
In second case study in which an old man is suffering from dementia, it may have significant
impact on social relationship. Further, disease can be overwhelming for members of family and
for their carers. Emotional, physical and financial pressure can also cause high stress to family
and support is essential from legal, financial and social system. Further, in my view disease also
have direct impact on the ability of person to remain socially engage. There may be negative
impact from social changes that are being attributed to factors like choce rather than dementia
(Joseph, 2015). Taking care of the person who is suffering from dementia is very challenging.
Social impact of dementia involves loss of employment, loss of social relationship, there is a
requirement to relocate the living arrangements in order to provide care to the patient. There are
various health impacts on person suffering from dementia like stress, sleep disruption etc. It is
imperative that the person suffering with dementia does not face any difficulties and this can be
done only when they are given adequate support and that too for the entire or total number of
hours.

I have supported the people who have suffered from accident in different ways. Feeling
emotionally, socially and physically unsafe may cause high level of anxiety in person who have
experienced significant life event like car accident or suffering from disease like dementia. I
have made various efforts for developing a safe environment for successfully engaging the
patient in their care. I help other people that are involved in providing care to the patient like
keeping bathrooms, keeping noise level etc. I have adopted the person centered approach for
providing care to people suffering from dementia, who have already suffered from trauma like
car accident etc. Family members of the patient help me to account for individual need and
preference of the person who have experienced significant life events.
People have provided their consent for care and support. I have also ensured that services
will support the priorities, goals and aspirations of the patient. It helps to address social care

related with quality of life and the wider well-being of the person in addition to practical support.
Further, I have supported other practitioners to deliver care and support through coordinated
efforts of group of workers like health care practitioners, social care practitioners, home care
workers etc. I have provided a copy of home care plan that helps to meet out the needs of people
who are suffering from significant life event like dementia, car accident etc (Bohart, 2017).
The care diary includes information regarding care and support provided to the patient
and also highlighted needs, experience and preferences of the person. Further, I have also
recorded any type of change in the behaviour of pservice user. I have taken into account the
difficulties that people are suffering in performing their tasks. Example – Person suffered from
accident required support in eating and drinking, difficulties in communication etc. Moreover,
after one month I have taken feedback in respect of the patient that what type of improvements
are there in their day to day activities, behaviour etc.

P3 Comparing how external agencies and identifying the differences
Hospice and Age UK provided patients motivation as well as financial support to the
company. They provide them with various facilities that will help them to guide them to feel a
motivated about their life. Voluntary agencies helped them to view the life in positive sense.
External agencies-
Social service- it helps to improve the mental ability and disease of people, social
services provide appropriate assistance to person and their families through health care
programs.
Nurse- group of nurses are closer with patients rather than any other health care
professionals. They took proper care of people and give essential support to their family
members as well.
Dietitian- they play vital role in well being of human health, dietitian provide guidance
to individual suffer from dementia, they inform and teach them about taking nutrition and proper
diet.
Voluntary agency-
Hospice- as compare to statuary agency, voluntary agency performs better because they
only focus on using new techniques and methods which they can use to improve health of
patients.

Age UK- it is one of the registered charities in United Kingdom where they perform for
human well fare and social care, as compare to dietitian Age UK serve in wider area of
community. They take expert advice for providing health care treatments to individual when they
get injured or face dementia disease reaction.

Barriers in the partnership of external voluntary and statutory agencies: Wile operating
in healthcare industry, when these two organisation’s meet, things are bound to go wayward and
therefore, there are a lot of barriers in the partnerships and these can be detailed in following
points:
Conflict in Roles: The voluntary care takers and professionals that have been appointed
are much more involved in the entire process as they have been hired and are paid to do the work
for which they have been assigned and therefore they work in a dedicate manner where they
perform three duties extremely carefully and diligently. On the other hand, social workers are not
too much involved since they have to attend to the requirements of a variety of patients and
therefore, it gets difficult to coordinate since the level of dedication and quality of service
provided varies between both the parties.
Communication and Networking: While communicating and networking with each
other, there are a variety of issues that impact the ease and flexibility with which both parties can
operate. Therefore, the diverse backgrounds, separate beliefs, different training procedures are
some of the key aspects while integrating and operating with each other. Therefore, statutory and
external voluntary care takers face lot of barriers while working with each other.
Role of statutory agencies-
Social service-
They are mainly accountable for managing, evaluating and providing social care as well
as support services, the role of this statutory agency is to assess what degree of support and care
is right in any given situation.
Nurse-
The role of nurse is to care and advocate for patients of all religious background and
ethnic origins and support people through illness or health.
Dietitian-
the role of this statutory agency is to promote nutrition in nursing home and hospitals to
improve the health of patients.

When an individual is suffering severe diseases they are going through both mental and
physical state. They are in need of person who can support them in this hard time it is essential

for them to have a health assistant. In the context of both the case these roles assistant can play a
significant role in motivating the patients to recover faster. Assistant supported them by listening
to their grievances that give them feeling of belongingness. Assistant provide them wit balanced
diets that help them to recover faster and feel energized. Balanced diet is essential in both the
cases. If they are not provided with balanced diet this will move into depriving stage. They
cannot be left alone in such situation as they may do something wrong with their life. Assistant
read motivational and inspirational biographies of various people for encouraging them change is
possible and one should not lose at any stage of life in any critical stage. Assistant provided
every facility that was required by them. The recovery of patients started faster after receiving
proper assistance and care from the assistant worker. Assistant and patients developed strong
understanding between them as continuous motivation developed health relation between them.
Working as domiciliary care worker I give my hundred percent and full efforts in context of
both persons how are facing some issues due to different incidents in their life. In context to first
case I took care of patient who get injured of leg and arms, they give them support through care
takers who always be there from individual. In regard to second case, I provide reminder clock to
patients who suffer from gradual cognitive decline.
As domiciliary care worker I had addresses some barriers on external agencies'
perspective that impact on patients and their family. Lack of effective communication is one of
the barrier in perspective external agencies that affect service users and their family members. In
my opinion this is one of the biggest challenges faced by patients that impact on their health
recovery and treatments.

National Health Service (NHS) -
It was founded in year 1948 in United Kingdom. It involves NHS Scotland, NHS
England, NHS Wales etc. The parent organization of NHS is Department of Health and Social
Care. The website of organization is https://nhs.uk.
Confidentiality :
The duty of confidentiality of healthcare professionals arises when one individual
discloses information to another individual in situation in which it is normal to expect that the
details will. For example- patient to clinician. The information of patients is normally held under
ethical and legal duties of confidentiality. The policy of NHS is that information related with
patients suffering from significant life events should not be disclosed or used in a way that may
identify the patient without their confidence (Bradley and Tulloch, 2017).
The professionals working in NHS are aware of the details disclosures that may take
place while providing practical support to patients suffering from significant life events.

workers plays an important role in providing appropriate care to the patient such as individual
suffering from accident or suffering from dementia etc. Domiciliary care workers are specially
trained to assess the needs of patient and the services they offer that best suits them. These
people also help people suffering from significant life events if patient just require to talk things
through. Social workers are also known as care managers that supports patients and their social

group like family, friends etc. in social service departments, care homes or hospitals (Burger and
Samuel, 2017).
Further, they also provide support to patient and their family by identifying right way to
access the funds for fulfilling the need of expenses for the treatment etc. These people are
primary contact for requesting continuing funding for the purpose of care services. Further, as a
care worker it is an important function to firstly identifying the role-played by each family
member in the life of individual suffering from significant life event. Moreover, I have identified
the boundaries and also focus on learning how the family communicates (Lobelo and de
Quevedo, 2016.). I have supported the patient by explaining to them how to communicate their
feelings to family members and other social groups. This is a very challenging task. I have made
efforts to ensure there is proper communication between families and patient which is an
important part of assisting the social groups and families to overcome with the suffering of the
individual. Another major role-played by social worker is by acting as a care coordinator. They
helps in developing link between doctor and families and communicating with service providers
to ensure care is provided in consistent manner to the patient suffering from significant life
events (Naslund and et.al, 2016). Another thing is that, care workers plays a major role as an
advocate by helping the families of patient to understand the requirements of their loved one and
also ensure that they will receive adequate level of care and support. Moreover, I have adopted a
impartial point of contact for the family members of the patient. Due to different family
dynamics sometimes individual feels that they are not being heard by their family or friends in
that case, care worker supports the patient in explaining their stress and concerns. They play the
role of surrounding board and assist the patient through stresses and concerns.
P 7) Referral process available to support individual and social network.
Referral process -
The term referral refers to the transfer of care for the patient from one clinician to another
person by request. Tertiary care is generally provided to the patient from primary and secondary
medical care personnel. Under patient referral, patient is directly inform their partners regarding
the exposure to the infection.
Community based services refers to the system of care that develops not only the
strengths of patient but also focuses on strengths of the community in which the family of the

patient lives. The referral process available to individual suffering from significant life events is
described below-
Develop list of available services-
Firstly, information will be collected regarding available services and programmes from
various sources like some healthcare agencies may already have that type of list (Vermeir and
et.al, 2015).
Note eligibility requirements-
Information will be collected regarding eligibility requirements for the care services.
Example- eligibility requirement based on diagnosis, age etc.
Determine caregivers and patients need for services-
Need of patient for the service will be determined. If patient is actively identifying the
need then there are more chances that they will act upon such referral.
Identify eligibility of patients for specific service-
Before making referral, the eligibility requirements of the agency will be reviewed to
ensure that patient and family is eligible for the service.
Be aware of ability of organization to provide services-
Be more realistic regarding the ability of organization to provide such services. Does the

I have provided support to people experiencing dementia and who have experienced
accident in different ways. I have provided service of personal care such as changing bedding,
dressing, washing, doing laundry and helping the patients with meals. I have supported these
individuals by firstly assessing their needs and requirements (Godinho and et.al, 2018). I have
also supported the families of patient suffering from significant life event like dementia by firstly
understanding the dynamics of their families. The support provided by me to the patient is very
effective in assisting the patient to explain own issues to family members. It also resulted in
emotional recovery of the patient.

CONCLUSION
It has been concluded that Further, the report has described that significant life events
like car accidents, disease of dementia etc significantly affects the life of individual who have
suffered or suffering from the event. Further, it also impacts family and social relationship of the
people. Such type of events affects the mentality and behaviour of the people that are
surrounding the people who have suffered or suffering from the event (Nelson, 2018). The
Report has also explained that, it is general that society and family members reacts in different
ways in respect of the patient. Moreover, adopting a person centred approach helps to provide
care and support to the people.
