INTRODUCTION
Present essay is particularly based on the case scenario given of a person whose name is
Mr. Sylvester Charles. An old gentleman, with an age of 58 years living with Susie (Wife).
Individual was the Senior Manager of a well-known insurance company, but due to rapid
changes that company was continuously making, it has become one of the reason for increasing
within the distress and raised issues for Sylvester. This is where, he took the retirement and
started tending to the garden. Including this, one day Sylvester found collapsed within the garden
area, where the reason that came in front was the stroke.
Away with this, essay will put light on activities of daily living; ADL’s by Roper-Logan-
Tierney Model, Waterlow score, NEWS2, VIP score. Also, it will also put light on critical
analysis over the cause of the two identified nursing problems. Including this, it will also put
light on pathophysiology and may also include all the applications as well in order to put relevant
information in relation to pharmacological/non-pharmacological interventions. Lastly, a care
plan will be developed for Mr. Sylvester Charles, which will help him in getting recovered as
soon as possible.

Roper-logan-tierney's model for nursing based on a model of living:
The Roper-Logan-Tierney Model for Nursing is a theory of nursing care based on
activities of daily living, which are often abbreviated ADLs or ALs. Within United Kingdom,
this model utilised by public sector business organisations like NHS and so on, which was
developed by Winifred W. Logan, Nancy Roper along with the Alison J. Tierney. Basically, this
theory could effectively lead Mr. Sylvester Charles to get healthy again as, various health
practitioners has often started utilising different ways through which, how the life of an
individual who is sick got changed because of illness, injury, or admission to a hospital rather
than as a way of planning for increasing independence and quality of life. In the same way, if it
is talked about Mr. Sylvester Charles, then he has been dealing with number of issues like
hypercholesterolemia, hypertension, they gave up smoking 12 months ago and diabetes type 2 as

implementing process in relation to provision of care given to Mr. Sylvester Charles would
become much more easy and improvised as well.
It is also critically analysed that not only, maintaining a safe environment within the
Hospital is required but also it is much needed by Susie to develop a much more healthy
environment as well based on the situation of Mr. Sylvester Charles. This could be understood
with a good example where Mr. Sylvester Charles left gardening his gardern. This is where, it
was pretty much required by Mrs. Susie Charles to deliver the right support through considering
all the aspects like eating along with drinking, maintaining a safe environment, personal
cleansing and dressing, breathing, mobilising, eliminating, playing and working, controlling
body temperature, expressing sexuality, sleeping and dying stage as well. Here, it is said that
Susie would require to deliver the right path to Sylvester to clearly gain a number of focus over
different areas where, he may not feel depressed at all.
Discussion and critical analysis of the cause of the two identified nursing problems and factors
that influence the care planned for the patient
Mr. Sylvester Charles was the individuals who was dealing with number of problems in
relation to stroke, anomia, aphasia along with the history of hypertension, diabetes type 2 and
hypercholesterolemia. Including this, if it is talked about the two different nursing problems that
are being taken into consideration are:
Acute impaired physical mobility related to the inability to move the right side of his body
as evidenced by left-sided ischaemic stroke with difficulty in walking. This is considered to be
one of the crucial problem, which was faced by Mr. Sylvester Charles and his wife because, this
paralysis attack impacted upon overall processes that Mr. Sylvester Charles used to do, which
raised different questions in mindset of Susie. Basically, a depressed person who has already lost
his hope of living the life happily cannot survive for a long period of time, even through offering
