Description of the stakeholder Map: The stakeholder is divided into four quadrants
known as internal operations, external operations, internal executives, and external executives.
The details of all the stakeholders are described below.

Internal Operations: The internal operations rea carried out by the Administrators, CEOs or the
system manager (Squires & Anderson, 2015). Internal operation involve maintaining the system,
launching the system and validating the system.
External Operations: These are activities in the system that are carried out by external
stakeholders that involved the doctors, service providers, and the hospitals (Case et al., 2015).
The doctors provide treatment process to the desired patients, which is considered to be an
external operation. The service providers provides system for storing the data of MY HEALTH
RECORD securely.
Internal Executives: The internal executive has the involvement of the patients and the
employees who are associated with the system of MY HEALTH RECORD (Whittington et al.,
2015). The patient are the most important part of the health care system who upload all the
details of their health on the system. There are also employees involved as the internal executives
of the health care system who helps to resolve the difficulties that are faced by the patients.
External Executives: The External Executives has the involvement of the government, the
equipment supplier and the medicine supplier of the system. All the investors who are involved
with the system of MY HEALTH RECORD is also considered as the external executives of the
system (Catarinucci et al., 2015). As government is a trusted body among the citizens, the
government can promote the system of MY HEALTH RECORD for common use.
2. Questionnaire
This section consists of a set of ten questions that can be asked to a stakeholders group,
which falls under the Internal Operation quadrant. The internal operation group of stakeholders
consists of people such as admin, CEO, and managers. They are to know all the questions

regarding the system of MY HEALTH RECORD system and many questions can be asked to
them related to the security of the system, usage of the system, advantages as well as
disadvantages of the system of the online healthcare system. The set of ten questions are as given
below:

people so that the people can get aware of the online process?

the patients stored in a system that can be viewed by all doctors and
hospitals all over the world. If the patients case is critical or needs and
emergency service, the hospitals involved can get contact that particular

patient for providing an emergency service. The triggering event of this
system is that the patients can get prompt service from the website of MY
HEALTH RECORD.

Description
MY HEALTH RECORD is an online application that is used by patients
to store the data about their health and fetch help from the system. MY
HEALTH RECORD consists of doctors, hospitals and administrators who
helps the patients to solve their problems (Nelson & Staggers, 2016). This
is basically an online website that can be viewed by the doctors, hospitals
and the patients all over the world so that critical to critical cases can be
solved. The system stores the information of the patients securely and
notifies them when someone helps them.
Actors The actors involved with the system are administrators, patients, doctors,
and cloud providers.

The stakeholders in all the quadrants of the system are managers, admins,
CEOs, doctors, hospitals, and cloud providers, government, medicine
suppliers, investors, patients, employees of the system.
Pre-conditions The detail information of the patients are to be stored in the system for the
doctors and the hospital to view information about the patients. Details
must include the patient’s current status, previous health history and the

contact details of all patients enrolled. There must be details of the
hospitals and doctors as well so that the patients can contact them
personally (Bates et al., 2014). The payment option for the system of MY
HEALTH RECORD should be made flexible with online payment system
with debit cards, credit cards, and net banking facilities. The system
should also have an option of providing insurance to the patients in case
of huge amount of money that is not possible for them to pay.
Post-Conditions The post conditions deal with the treatment process. After entering the
details of the patients, the admins and the mangers should keep a look on
the treatment process. The address of the patients, hospitals and doctor
should be valid. The payment process of the system will have to be
successful for proceeding with the rest treatment process (Wager, Lee &
Glaser, 2017). The card number to be given by the patients should
validate in order to make a successful payment. Even if the treatment
process is completed, the data of all the patients so far is to be stored on
the database of MY HEALTH RECORD securely for any emergency
future use.
