1.1. Description
Case Worker- A case worker is appointed to each of the emergency department of the
hospital and communicate with the young people for finding their problem. The mental ill health

condition of the patient are recorded by the case worker and they also assist the patient to record
their story,
Medical practitioner- The medical practitioners analyzed the story of the patient and is
assigned for providing a proper diagnosis of the patient health and mental condition.
Patient- The patients are mainly the young people and they are involved with the
treatment process and uses the healthcare system for getting advanced treatment. The result of
the diagnosis of the patient health condition is required to be available for the patient.
Project sponsor- The sponsor of the project is not directly involved with the project
development project and they should be informed for making any change in the information
system.
Insurance Organization- the patients treatment are insured by some of the patients and
thus the insurance company is require to be involved in the stakeholder map for getting the
treatment amount.
Government- The government is included in the stakeholder map for maintaining a legal
standard in the treatment procedure. The devices used in the treatment are standardized and
mitigates the risk associated with the development of the system.

determination result and the story under the
patient_ID.

Description The patient needs to register into the
system and a user account is required to be
created for recording the story of the patient.
The details of the patient such as the
patient_ID, record ID is required to be
included for the development of the
information system. The case worker assist
the patient for recording the story and
minimize the risk of error input of data in the
system.
