2. Questionnaire
2.1 Stakeholder Identification for Questionnaire Management
A questionnaire survey is to be conducted in order monitor the working of the My Health
Record System and understand how this new system is benefitting the operations at the hospital.
Hence, the questionnaire will be targeted towards the stakeholders who fall under the internal

operations category and include Admin, CEO and System Managers. These stakeholders will be
connected to the system throughout their period of work and hence, they are most likely to
determine issues in the system as well as the benefits they are receiving compared to the previous
systems.
2.2 Purpose of the Questionnaire
The purpose of this questionnaire is to determine the benefits received from the “My
Health Record” system as well as identify any issues that might be faced by the stakeholders
while using the same.
2.3 Questions to be Included in the Questionnaire
As a part of the questionnaire survey process, the following questions will be included.
There have been a few months since MY HEALTH RECORD has been implemented.
Have you noticed any improvements in the hospital operations? If so, what are they?
Please explain the working of the system as you have experienced.
Have the patients reported any issues from their end like facing problems while
uploading their documents in the online portal?
Which cloud operator is used for storing the documents of the patients and the doctors?
Have you found any issues in the MY HEALTH RECORD system? What are they?
Do you think the documents stored in the cloud of MY HEALTH RECORD system are
secure?
Have the issues reported by the patients been resolved at the right time?
How do you plan to enroll more doctors and patients to use the MY HEALTH RECORD

The system that is described for the Tele-Health Record helps to get all
the information that are involved in the system of Headspace. The
information that are stored in the system includes the details of the
patients, doctors, and the employees who are involved in the system. If
the patient needs emergency service from the Headspace, the organization
is able to provide the service to the user. The triggering event that is
involved with making the system online is that the patients can get very
quick service from the online service of My Health Record.
Brief
Description
The application that is developed for My Health Record includes the data
of the patients that are stored in the data base of their system. There are
doctors, administrators, nurses who are involved in the system who are
involved in solving the health problems of the patients who registers in to
the network. This application that is designed helps to view the details of
the patients which are needed by the doctors. The data are stored securely
in the system database by the cloud provider.
Actors The actors who are associated with the online system of My Health
Record includes patients, cloud providers, administrators, and doctors.

Similar use cases are being studied for designing the health care system of
My health record.
Stakeholders The stakeholders that are associated with the online healthcare system are
divided in to four quadrants. The quadrants are known as internal
executive, external executive, internal operations and external operations

executives. The stakeholders involved are government, medicine
suppliers, CEOs, hospitals, investors, patients, managers, cloud providers,
admins, employees, and doctors of the system.
Pre-conditions The information of the patients are to be stored in system of healthcare so
that the doctors can treat them accordingly and prescribe the patients
accordingly. The details of the patients includes the current status of the
patients, history of previous health related issues and contact details of the
patients. The System of My Health Record should have a flexible
payment system with debit cards, credits cards, and also should have net
banking facilities.
Post-Conditions The post conditions that are included in the system is the treatment
process of the My Health Record. The admins who are associated with the
Headspace should look after the treatment process whether the treatment
process is going right or not. There should be a proper address and a
contact number of the patient so that the patients can contact them when
necessary. In post condition, the payment process should also be
successful after the treatment is completed.

securely stored.
3.2 The login details that are available should be
correct and validate by the admin of the system.

Condition
The exception that can arise for the system is that there can be difficulty
in contacting the patients if the contact details of the patients are not
uploaded correctly. This case may arise if the patient changes his contact
number.
