Note that research solely involving analysis of existing data (secondary analysis)

HOW TO COMPLETE & SUBMIT THE APPLICATION
1. Complete this application form electronically, fully and accurately.
2. Type your name and ‘student’s signature in the designated area.
3. Include copies of all necessary attachments.
4. Email your supervisor /lecturer the completed application and all attachments as
ONE DOCUMENT. INDICATE ‘ETHICS SUBMISSION’ IN THE SUBJECT
FIELD OF THIS EMAIL so your supervisor /lecturer can readily identity its
content. Your supervisor will then look over your application.
5. When your application demonstrates good ethical protocol your supervisor will
type in his/her name in the ‘supervisor’s signature’. Your supervisor /Lecturer
will let you know the outcome of your application. Recruitment and data
collection are NOT to commence until your LSME ethics application has been
approved, along with other research ethics approvals that may be necessary.
