Title: Teaching and learning plan for professionals to understand Chronic Disease Management
Target audience: This includes general practitioners (GP), diabetologist, diabetes specialist
nurse, counsellors like podiatrist, dietitian etc. with endocrinologists
Topics: Basics of Chronic disease management including definitions, symptoms and effects, use
of Observations of Daily Living (ODLs) for promoting self care among the patients, Key
performance indicators to implement the treatment plans and other use of medication. Self
monitoring aspects to maintain the well being of affected chronic diseases patients.
Objectives: To impart the significance of self care for improvements in the clinical care for
chronic disease management
Utilisation of available resources: Power point presentation would lay the foundation of all the
information and details about the topics to give the audience an overview. It further highlights
the key features of this teaching plan through using visual materials and use of poster which
would give a better understanding to the chosen professionals and using interactive sessions for
exchange of information and data.
Teaching strategy: This strategy includes use of visualisation via making a power point
presentation to impact their minds and establish a robust approach to promote the use of person
centric approaches for better management of chronic diseases like diabetes. Use of charts,
statistics and variety of published materials would act as an asset to initiate the interactions
between the teachers and the professionals chosen for this plan. Herein, the nurses and
Healthcare Providers (HCPs) would be given a platform for generating awareness and promoting
collaboration to mitigate the rise of cases in this area (Schulman‐Green, Jaser, Park &
Whittemore, 2016). Moreover, this would pave the path of analysing the problem areas, one to
one interactive sessions to improve one's knowledge and clear doubts. Nevertheless, its learning
role is passive and tackle the domain of cognitive behavioural approaches through asking
questions.

Evaluation method: This is done through application of feedback and forms filling after
conducting each session which would give clarity and transparency on the complete
effectiveness of this teaching plan (Mirowsky, 2017). Along with, the focus area is to gain
insights and address the queries in proper manner and this would be evaluated through taking all

and lies between 22-40 years. Their active participation would bring fruitful changes through use
