Identification of group of individuals or individuals for chronic
disease management (CDM): The education session is going to be held
for people working for mitigating the effects and measures for chronic
disease management of diabetes and includes general practitioners
(GP), diabetologist, diabetes specialist nurse, counsellors like
podiatrist, dietitian etc with endocrinologists. These together support in
managing the levels of insulin in affected individuals and help them to
minimise the impact on the wellbeing of their bodies and overall fitness.
Identification of managing a chronic condition for targeted
audience: Promotion of Self-care to enhance the clinical care by
imparting education about the Observations of Daily Living (ODLs) is
one of the important determinants to manage any chronic disease among
the Canadian populace.
Factors that influence the learning and teaching process in Chronic
Illness in Canada:
Canada is one of the astonishing countries which are completely
vigilant about various aspects related to the health and social care
sector. Herein, there are several parameters that must be looked into to
understand the onset of chronic illnesses in terms of learning and
