performing in team are more efficient as compared to doing their work in individual manner. It is
considered that if team work is done to handle the patients suffering Dementia then it will surely
increase the health performance of patients.

Dementia is the general term which is used to take loss of memory and other mental
abilities which create problems to run the life in proper manner. If general practitioners deliver
care and proper level of health care program for the patients suffering from dementia then it will
increase their health and reduce the illness with the certain limit. Team approach in improving
the health and quality of life of people suffering with the dementia is concerned with a group of
individuals, health doctors, general practitioners and others persons. If team work is done that
these patients could be imparted proper level of training and look after their health. These health
care services should be in coordinated in proper manner (Duckett & Willcox, 2015).

There are several approaches which could be used for caring people with dementia to
make their health better.
Person centred approach- It is the teams working effective approach in which people with
the dementia are provide proper centred level of services in health care department after
considering proper level of value, belief and perception of the patients.
Compassion in healthcare- This level of health care services are more inclined towards
hiring doctors and GP who are passionate to take care patients in effective manner. This will
increase the overall outcomes better health of people with dementia.
Recovery based care approach- This level of health care approach will allow all the GP to
undertake all the possible means to improve the health and mental level of people with dementia
in better manner. It will reduce the symptoms of people with dementia to improve their health.
Trauma-informed care- This care approach is implemented when the people with
dementia is at his last stage. This health care approach will help patient to recover from its ill-
ness in effective manner (Zimlichman, et al. 2013).

Health professionals play a centric role in improving access and quality health care for
populations.
Role of health and social care workers
They provide crucial support and physical care for the betterment of patient’s health.
These social care workers help patients and family members to understand the system
of hospitality and improving the health of people with dementia (Blumenthal & Hsiao, 2015)
They will also keep the record of the people with dementia and will maintain it on the
public health care system for their better treatment.
They help people with dementia to improve their health and their eating habits in
effective manner.
They improve the quality of life and establish proper level of coordination in health care
services.
Responsibilities of health care professionals
The main responsibilities of health care worker are to improve the health of the people
with dementia and provide best level of look after services. They are responsible to show people
with dementia a right path to maintain effective life balance.
