SCENARIO 3
What care planning models are you aware of?
There are several models which is applied in nursing. By that an effective care plan is
developed. So, Orpet and Jeffery ability model contain various stages. Its purpose is to
understand patient need and address it. Also, ability assessment is done in it. Moreover, another
model is Orem theory of nursing (Barker, 2017). There are 3 theories in it that are self care, self
care deficit and nursing system. Thus, both these models enable in effective care planning.
How does this relate to the nursing process?
The models relate to nursing process as on basis of care plan, health care services are
given. Moreover, on basis of model entire nursing process is identified that what activity is done,
when, how, etc.
Discuss how you used these to develop and implement a plan of care?
I used the planning model to find out what are needs of Natalia. Then, I analysed that
what problems is being faced by her. So, on basis of it I took certain measures and implemented
plan in effective way.
Who else needs to be involved in the care planning episode?
It is found that Natalia family and school friend’s needs to be involved in care planning.
Also, psychiatrist must be involved in it by which an effective care plan is developed.
Analyse the skills required for therapeutic relationships across the lifespan.
It is necessary to possess relevant skills and knowledge to provide care services to
Natalia. Also, it is will enable in ensuring high quality care services as per her needs. Hence, for
present case the most crucial skill required is communication. This is because it is necessary to
effectively interact with Natalia so that her needs are identified. It will allow her to be
comfortable so that she share her feelings, opinions, etc. (Edward, and et.al., 2017) Along with it,
another skill required is shared decision making. It will enable nurse to interact with Natalia and
make effective decision. Furthermore, by consulting with Natalia mutual decision will be taken.
So, on basis of it care plan is developed. With that it will be easy to develop strong relationship
with patient and provide therapy.
What is compassion?

In health care sector compassion means to show empathy, respect and dignity to patient
while giving care services. It is ability to understand situation of patient and to take relevant and
effective action to improve life.
What ethical considerations are there when undertaking care planning?
It is important to take several ethical consideration in care. This ensure that care services
are delivered with ethics. Basically, there are 4 ethics which is followed in it that are autonomy,
justice, beneficence and non maleficence. These all allow in taking right decisions. Moreover,
here Natalia will be having right to take her own decision based on her ethics and values. In
addition, there should not be any personal interest involved of nurse in providing care services to
patient. Besides that, there is need to maintain privacy and confidentiality of data of patient.
Hence, by following of these ethics care planning is done (Ellis, and Alexander, 2017).
What professional considerations are there when care planning?
Alongside, ethics there is need to take some professional consideration as well in care
planning. Here, it ensure that care plan is properly developed. Hence, consideration is informed
decision making that what decisions are taken and entire process of care services. It is important
as it aids in empower patient and overcome low health literacy. The patient get to know how
their health will be improved. Another is telling truth in which patient is given all info which is
true. There is no false information added and communicated to patient.
Critically reflect upon your personal and professional development in clinical practice.
Here, it is clearly seen that Natalia has taken overdoes of paracetamol. So, she require
proper care. In this first of all I will try to get comfortable with Natalia by interacting with her. I
will ask her to share feelings. Then, I will identify needs of Natalia. Then, accordingly I will treat
her. Besides that, I will ensure that she get engage with other friends. Then, I will develop a care
plan on basis of which treatment is given.
Here, I will use Gibbs model of reflection so that I will be able to evaluate my
experience. So, as there are several stages in it so I can easily analyse my performance. In
description I will describe what happen, where, when, etc. Also, I will share my feelings and
what other people felt during event. Thus, I will identify what went well and what not. So, with
help of analysis and conclusion I will be able to develop action plan that in which areas I need
improvement. Therefore, Gibbs model will help me to analyse my behavior and actions during
care given (Holmberg, Caro, and Sobis, 2018).

SCENARIO 4
What care planning models are you aware of ?
There are several models which is applied in nursing. By that an effective care plan is
developed. So, Orpet and Jeffery ability model contain various stages. Its purpose is to

How does this relate to the nursing process ?
The model of care plan relate to nursing process as plan gives insight on what type of
care is to be given. Then, on that basis overall nursing process is formed. The process vary on
basis of care plan and measures specified in it.
Discuss how you used these to develop and implement a plan of care?
I used process to develop plan as I found that what are patient needs and how it can be
met. Also, I got to know about steps taken in it and critically thinking what measure can be
included in care plan (Oates, Drey and Jones, 2017).
Who else needs to be involved in the care planning episode?
There are many people to be involved in it. First is Susan husband and her friends. Also,
members of community mental health team must be involved. So, it will enable in developing
effective care plan. Moreover, proper measures will be taken in it.
Analyse the skills required for therapeutic relationships across the lifespan.
There is need to have certain skills for therapeutic relationship with patient. This is useful
in providing care services. In this scenario it is found that skills required is family centred care.
This is because it will help in working with Susan friends and relatives to provide health care
needs. Along with it, another skill required is collaborative working. As it require to work with
closely with Susan. Also, by working with community mental health team strong relationship
can be developed with her. Communication is also required to effectively interact with her. This
is because it is necessary to effectively interact with Susan so that their needs are identified. It

will allow to make her comfortable so that she share her feelings, opinions, etc. (Patterson, and
et.al., 2018)
What is compassion?
It refers to show empathy, respect and dignity to patient while giving care services.
What ethical considerations are there when undertaking care planning?
It is essential to follow ethics in care planning. This ensure that plan is developed in
ethical way. Hence, ethics considered are autonomy, justice, beneficence and non maleficence.
Here, Susan will be given autonomy to take decision in regard with her care. Also, beliefs and
her dignity will be maintained in it. Moreover, the past history and record of her will not be
shared with any other person. Besides that, no action will be taken which can cause harm or
injury to her. There will be fair decisions taken in giving treatment to Susan. Also, all laws and
regulations will be followed when making choices or decision. So, these all ethics will be
considered in care planning (Slemon, Jenkins, and Bungay, 2017).
What professional considerations are there when care planning?
In present scenario, some professional consideration is taken in care planning. So, first is
that informed decision making that what decisions are taken and entire process of care services.
It is important as it aids in empower patient and overcome low health literacy. The patient get to
know how their health will be improved. Another is telling truth in which patient is given all info
which is true. There is no false information added and communicated to patient.
Critically reflect upon your personal and professional development in clinical practice.
By analyzing case study it is found that Susan is diagnosed with Bi polar disorder. So,
first is I will develop a care plan by communicating with her. In that I will monitor mood of her
which will help me to find out right treatment. With that I will determine her needs. For that I
will interact with her family member, husband, past health records, etc. Hence, with that I will
try to treat Bi-polar disorder (Patterson, and et.al., 2018).
Here, I will use Gibbs model of reflection so that I will be able to evaluate my
experience. So, as there are several stages in it so I can easily analyse my performance. In
description I will describe what happen, where, when, etc. Also, I will share my feelings and

improvement. Therefore, Gibbs model will help me to analyse my behavior and actions during
care given.
The quality of care delivered was not so high (Townsend, and Morgan, 2017). This
means that I did not took human factors in it that is Susan children. Also, the effectiveness of
care was not high as I did not set certain criteria on basis of which care services is delivered.
