INTRODUCTION
Health assessment is a plan that outlines the specific needs of the individual and how
those needs could be addressed by health care system. According to the constitution of India
health care is the responsibility of the state governments to raise the level of nutrition and the
standard of living of individual by improving public health services (Zeanah, 2018). The present
assignment focuses on the primary, secondary and tertiary health care services that caters the
needs of the individual. It also elaborates Health watch and Clinical Commissioning Group.
1.Definition and an example of the following
Primary Care
It is the day to day health care facilities provided to the individuals according to their
needs. These are the providers who act as a first contact for the continuous care of patients and
coordinates with other specialist according to the patients needs. Primary care providers are the
doctors, nurse practitioners and the physician assistants. Primary Care Support England is the
primary care service provider in UK which fulfils needs of the individual.
Secondary Care
It is the medical care facility that is provided by the specialist by the referral of the
primary care physician. It requires more specialised knowledge, skills and equipments than the
primary care health providers. It is the specialist focus on the patients specific system of the body
or a specific disease or condition (Estes, 2013). This is the condition when primary health care
providers are unable to find solution to the problems and end up referring to the secondary health
care providers. Clinical Assessment and treatment services provides excellent secondary care
services all over UK.
Tertiary Care
These are the specialized consultative care service providers which provides specialised
health care facilities by working in a centre that has a personnel and facilities for special
investigation and treatment. These are the physicians which are specialised in neurology,
cardiology and other specialised fields.
Therefore NHS Provides appropriate health care services in to meet the needs of the
individual. It is a publicly funded national health care services throughout UK.

2. Explain PHE
PHE is an acronyms which stands for Public Health England. It is an executive agency of
the Department of the Health and Social Care in United Kingdom. It came as a formation as a
result of the reorganisation of the National health services in England. Its main purpose is to
protect and improve the nations health and to address inequalities. Its employees mainly includes
Scientists, researchers and public health professionals. These greatly satisfy the needs of the
individuals at a larger extent (Koenig and et.al., 2012.). It has various health protection and
health improvement programmes such as healthy diet and anti smoking marketing as well as
measures to reduce health inequalities.
3. What is Health watch
Health watch England is a body which is established under the health and social care act
2012 and came under effect in April 2013. Its main purpose is to understand the needs,
experiences and concerns of the people who use health and social care services and should speak
out the benefits on their behalf. Therefore Health watch is a UK based charity which promotes
evidence based medicines and determines the assessment and testing of the treatments and
provides consumer protection of all the forms of health care through testing all its products and
procedures and provides better regulation of all the practitioners.
4- What is a CCG Clinical Commissioning Group
These are the NHS organisations set up the Health and Social Care Act 2012 to
reorganise changes in the services in England. It is a part of the government to create a clinically
driven commissioning system that are more sensitive to the needs of the patients. They are
responsible for providing with primary medical services by the members of the group and to the
individuals who resides in their area and are not provided with primary health care facilities
(Cronin and et.al., 2013).

The following are the health campaigns that addressed the needs of the patient and
increase awareness-
1. Mental Health Campaign- Campaign would be based on mental health which would be
hosted by mental health foundation and will take place from 13- 19 may 2019 and will

create awareness on the issues of stress on human beings and will create awareness on the
mental health problem in lifetimes.
Sustainable Health and Care campaign- There was a campaign related to eating
disorderedness which was scheduled from 25 the Feb to 3 march and will create
awareness of the issues in eating disorders and provides recommendation to the
individuals to improve their nutrition and diet chart.
CONCLUSION
It has been determined from the above report that health care is the primary need of the
individuals. To ensure this it is Government of UK has taken many initiatives and started many
campaigns in order to promote and secure health of employees. Government has opened up
many agencies so that every individuals can avail the service and take benefits out of it.
