consumers. All these three concepts vary on the basis of perception of an individual and
suffering from any kind of disease and take treatment (.DiClemente, Hansen and Ponton,
2013.). For these important concepts, organisation have to be proper knowledge of social care. In
case of NHS foundation care who are affiliated with that case to find out major cause of death of
patient, family of Tom got affected from their works and procedures. In that scenario Tom get
injured from the car crash. In health and social care, health represents mental and physical

that case when Tom injured in car accident so it directly affect on his health. According to the
