professional practice. They are pivotal in resolving any sort of issues. Clinical records document
data associated with the patient like test results, clinical findings, outcomes, treatment plans and
communications which are useful for clinical decision making (Ballantyne, 2019). Client records
like financial information and contact details are the key relevant personal data which are subjected
to the GDPR guidelines. Vet safe report are highly prominent because it helps in effectively
recording the mistakes, complaints and other internal investigation.
The key features related with the discharge and admission records mainly comprise of
preventative medical advice, signatures, nursing consultation records, name, pet ID registration
paperwork, address and insurance claim forms (Coates, 2015). The discharge report mainly
comprise of reason for the hospitalization, relevant treatment and procedure provided, significant
findings, discharge condition, family and patient instruction and the signature of physician.

difficult to handle the client in such delicate situation of grief and loss (Dunne and et.al., 2020).
The client is in intense sorrow, pain, grief, feeling of life has no purpose, depression, anxiety
attacks, feeling of detachment, numbness, etc. It is considered to be very crucial to verbalize the
feelings and seek complete help and support. The nurse is responsible for effectively managing
the pain and other symptoms associated with the loss and grief in clients.
