The current report is based on an accidental case of a well-known chemical plant in US
called Texaco Refinery. Herein, a series of certain undesirable events led to explosion and fires
on 24th of July, 1994. The most contributory cause behind this incident was found to be an unsafe
work permit that led to severe structural damage (Hosseini and et. al., 2015). An effective work
permit system is thereby referred to be a significant aspect for avoidance of such accidental
incidents. This is because a work permit system must involve safety assessment with an
emphasised need of reviewing the existent practices in terms of maintaining safety. However, in
the present case of explosion and fire in Texaco Refinery, there were several such technical
measures whose failure led to a drastic situation causing huge structural damages. This involved
modifying the plant with no prior or post assessment of its possible consequences.
Insufficient maintenance of instrumentation and plant has been found to be yet another
loophole in the work permitted system of Texaco Refinery. Apart from this, there were many
issues in the designing of control room that comprised an ineffective plant layout. Moreover, on
considering their emergency response, the emergency operating procedures were found to be
defective in terms of spill control (Omidi and et. al., 2018). Causes behind this was non-expertise
way of operating emergency processes. Besides this, there were untimely attempts for keeping
the unit in an operative state at the time, it should have been closed. The work permit system is
thus considered to be an important component of an organisation’s safety management system.
Its efficacious implementation is expected to assure a safe work surrounding with least
probability of injury to the workers as well the structural premises.
