1.
The registers or records used by the organization for collecting information on WHS
incidents include:
 Material safety data sheets

These records help in examining the information which can be used by the health and safety
officer in performing their duties under the model of Work Health and Safety Act. These
records help the officer in effective decision-making, identifying hazards, controlling risks
and improve the organizational performance (Board, 2019).

a) I work as an assistant HSE advisor in Lendlease, Australia that is a construction and
Infrastructure Company operating worldwide and the organization has 11,000 employees
until 2016. The company manages large projects of buildings, developing infrastructure, etc.
As an HSE advisor, it is my responsibility to ensure that the regulatory requirements are
fulfilled effectively; monitor contract safety procedures, improve HSE performance,
conducting internal and external audits, etc.
b) The workforce must be kept aware of the legislation and regulation of WHS prevailing in
the organization. This information must be communicated effectively by the managers to
their workgroup. Training and induction methods could be used to make the employees
aware of their WHS duties (Warmerdam and Lingard, 2017).
c) The issues discussed in the WHS meeting include inspecting or assessing the reports made
related to hazards, injuries or incidents at the workplace, planning effective methods for
providing WHS training to employees, etc.
d) The managers must identify, evaluate and select the most effective training methods that
are beneficial for the employees in gaining first-aid training, manual handling training,
hazardous substance training, etc. Also, the organization must undertake a health and safety
audit to maintain standards of workplace safety (Detsimas et al. 2016).
e) The steps to conducting training need analysis for the employees in the organization
include:
 Identifying desired business objective such as reducing injury, sickness, etc.
 Linking the desired objective with behaviour, knowledge and skills of the employees
 Collecting the data using survey, interviews, etc.

 Identifying and evaluating core competencies
 Identifying and understanding gaps in the performance of employees
 Setting priorities
 Identifying and selecting the methods of training to be used
 Undertaking a cost-benefit analysis

f) The information needed for conducting training needs analysis is to understand what type
of training is required by the employees and why, how many employees need to be
trained?, how and where is the training to be done? what will be the cost of training? And
how will the training affect the employees as well as the organization? The answers to these
questions are to be identified for conducting an effective analysis (Detsimas et al. 2016).
g) The WHS training required in Lendlease for the workers includes: safety awareness
training for the construction workers, GMR training for supervisors, first-aid training for the
line managers and workers, construction induction training for all the field workers, etc.
therefore, different types of training are provided to different types of workers depending
on their work role (Detsimas et al. 2016).

The potential work-related hazards identified in Lendlease are:
1. The workers who are engaged in the construction work at the field such as
electricians or engineers working with the equipment are at the risk of injury due to
electrical hazards.
2. Physical hazards: When the workers are engaged in the construction of the building,
they are at risk of accidental falls or the risk of falling objects due to vibration, noise,
etc. which may lead to injury.
3. The construction businesses are also prone to the risk of fire in the building,
premises, etc. which leads to huge loss to the business (Jeelani et al. 2016).

Methods used in Lendlease to identify hazards in the workplace are:
Pre-use planning: The Company focuses on conducting a test when new equipment is
introduced to the site.
Safety alert system: The organization uses detection technologies in which sound of the
alarm is added in the equipment as it rings when risk is sensed to a worker before they get
any injury (Jeelani et al. 2016).
Audits: Health and safety audits are conducted by the health and safety expert or officer
who undertakes an inspection to identify hazards in the workplace.

a) The current knowledge and skills of the staff should be assessed and the supervisor must
provide adequate training to the staff to develop their skills and potential. The formal
operating procedure for training the workers includes general awareness training and
operational training.
The operational training is provided to all the workers that are working on-site and handling
heavy machinery and equipment. The standards operation process include the review of
machine and equipment guarding procedure followed in the department, appropriate use of
personal protective procedures, training with the help of videotape and final training on the
field by considering appropriate safety measures to increase the effectiveness of the
workers (Horberry et al. 2016).
b) For the safe operation of machinery the workers should:
 Wear protective jackets when they are engaged in mining operations.
 The workers must always use the proper piece of machine and equipment on the
field.
 The electrician must take care of keeping the electric codes and cables clean and free
from kinks.
 The workers must wear a metallic guard while operating on any machine or
equipment in the field.
 The supervisors must focus on regular repair and maintenance of the machine and
equipment to eliminate risks.
 The workers must report to their supervisor immediately in case of any problem
faced with the machine or equipment (Horberry et al. 2016).
c) Policies for the annual shutdown period for full assessment and repair

• For full assessment and repair, the site will be closed in every alternative period of two
months.
Policies for maintaining personal protective equipment
• It is important to use the following PPE by the workers during work:
1. Shields or visors for eye protection
2. Earmuffs for ear protection from noises of blasts and mining
3. Hand gloves to protect hands from cuts and hazards.
4. Helmets to safeguard head from rocks and other accidents
• The worker must not misuse or damage the clothing or equipment.
• The PPE must be used in a manner in which he/she has been instructed (Tanko and
Anigbogu, 2012).
• Ensure the compliance of all PPE with current WHS standards and should be
stamped and labelled with an AS compliance marking.
• A regular reassessment of PPE should be performed to ensure its compliance with
the standards.
d) While handling hazardous substances, the employees must be provided with the required
knowledge of the number of hazards associated in the activity and what steps can be taken
in the workplace to reduce the risk or minimize the danger and avoid injuries that may occur
due to improper handling or storing of substances. The employees should take certain
precautions while using or moving the equipment such as wearing hand gloves, considering
eye protection, wearing steel-toed boots for safety, etc. The workers must be provided with
proper training before they go on the mining field to avoid accidents and use the materials
and equipment properly (Simpson and Horberry, 2018).

e) The first-aid procedure in the workplace includes:
 Identifying the type and location of the first-aid kit.
 Assigning the responsibility of the first-aid kit to an individual and checking and
maintaining it frequently.
 Ensuring effective training is provided to all the first-aiders.
 Ensuring that all the employees are provided with appropriate training and
instructions regarding first-aid facilities offered at the workplace.
 Explaining the workforce about how the injury is to be reported in the workplace.
 Providing counselling services to the first-aiders and employees in case of a serious
accident at the workplace (Nowrouzi-Kia et al. 2018).
f) Formal procedures for:
I. Identifying and reporting hazards:
The hazards can be identified by carrying out an assessment of risk, determining the factors
that are prone to risk for the workers, evaluating the risk and also recording and hazard on
an annual basis.
The hazard can be reported in the workplace by narrating the whole situation to the
supervisor verbally, completing the report form in written and raising the incident in the
team meeting or discussion (Modarres, 2016).
II. Conducting WHS audits and safety inspections
The formal process includes three steps:
Step 1- Answering a set of questions based on laws and standards
Step 2- Writing an audit report
Step 3- developing an action plan and implementing it to make corrections
III. Conducting risk assessments and implementing risk controls:

Action 1 monitoring the level of hazards, incidents and accidents through formal reporting
The first measure that I would take to address the declining WHS performance is to analyse
and monitor the level of Hazards and accidents through formal reporting. This would help
me identify the key areas and factors that are responsible for hazards and accidents at the
site (Vault IQ, 2017). Through this, I can effectively identify the key reasons behind the
accidents and make necessary improvements.
Action 2 providing proper training to the workers for using machines and equipment
I would focus on providing proper training to the workers so that they handle and use the
equipment and machines securely and avoid any risks and injuries at the workplace. Proper
training to the employees is the key to reducing the health and safety risk at the site.
Action 3 ensuring the use of quality machinery in proper working conditions
I would also ensure that the machinery and equipment used at the workplace are in proper
working conditions and accordance with the quality standards of the Health and safety
regulations (Vault IQ, 2017).
2.
The key area where the assistance of WHS specialist is required in the site is for consulting
and review of WHS management solutions at the site. The review and assistance of WHS
specialist in the WHS management solutions would support in identifying the shortcomings
in the WHS system at the workplace and formulating an effective solution to develop an
effective WHS system and reduce hazards and injuries at the site (NSCA, 2019).
