The aim of the document is to implement a new system for the mental health illness youth
named a New Access project. understand the problem in the previous assignment including
problems, capabilities and benefits of the system. it was also including some
recommendations for the system. this assignment is to gather the requirement for the system
like stakeholder map, questionnaire and use case diagram.
Australian government allocate a fund of $54 million for this project names as Headspace. It
is one of the organizations that is trailing the project for mental illness problem of youth. In
the role of a business system analyst, gather the requirement for the project and provide a

Stakeholder description is required to define the problems and their solutions for the basic
requirements of the system. it is required for basic system validation and the integration with
verification of the different activities. Define stakeholders in four different quadrants that are
required communication for the success of a project (Spacey, 2017).

questionnaire, both quantitative and qualitative data can be gathered. A questionnaire
provides practical knowledge with the easy analysis and validity & reliability of data
(McLeod, 2018). Create a questionnaire for the project manager of the current project as
below:

Use Case Diagram & Description:
Use case diagram is used for the demonstration of a system with different users’ interaction.
A UML tool is used to create this diagram that includes the scenario of the system or
application. Define different entities to achieve the goals of the system. it used to represent
the relationship of use cases, actors and systems (Smartdraw, 2019). Here presents the use
case diagram of the current system as below:
