1. Describe two (2) risk assessments that you would perform on Yun
There are several risk assessments in thrombotic cerebrovascular accident out of these
two majors in case of Yun are High blood pressure and obesity. As Yun is 79 years old man
and overweight with no physical exercise which result in CVA. Along with this, another
reason is that Yun have poor diet habit also which result in overweight as well as he takes
high salt in food due to which blood pressure issues taken place. For better and fast recovery,
it is important for Yun to change their living style because changes in such things results in
reducing risk of thrombotic cerebrovascular accident (Arshad, Iqbal and Baig, 2019).
2. Identify one (1) issue that Yun may have with meeting his activities of daily living needs.
Why has this issue occurred?
Yun have Impulsive issues which means that individual is in the condition which
person has trouble in controlling emotions or behaviours. He easily gets angry over things
while I am providing him treatment as well as this result in disturbing activities of daily
living needs. Although, such impulsive issues may create problem in responding to treatment
within positive manner. The best way of assisting Yun issues cognitive behaviour therapy
because with the assistance of respective therapy Yun will be able to control emotions.
3. You are required to perform a health assessment on Yun. Describe two (2) principles that
you will follow when performing a health assessment.
There are some principles of health assessment which I can use as care taker of Yun.
Explanation of these are as follows :-
 Inspection – It is important for care taker to inspect each and every body system by
using vision, smell, hearing to assess normal conditions as well as deviations. In
addition to this, assess for size, colour, movement, location, odors, symmetry as well
as sounds for assess each and every body system.
 Palpation – This physical assessment principle requires care taker to touch patient
with distinct part of hands by utilisation of varying degrees of pressure. Because hand
of care taker are their tools so it is important to short nails and warm hands (Bishawi
and et. al., (2018). Along with this, care taker have to wear gloves while palpating
mucous membranes or area in contact with body fluids. Furthermore, there are several
types of palpation such as light palpation as well as deep palpation.

4. Using a wellness approach to health, how will you ensure that Yun’s physical needs have
been met?
For thrombotic cerebrovascular accident (CVA) current standard treatment is drug
named as alteplase. In addition to this, tissue plasminogen activator has to be given through
vein within 4.5 hours of stroke onset (Valluri, Akhondi and Hyndman, 2018). Because it
provides assistance in breaking up clot and opens the artery so, that blood can flow easily to
brain tissues again. In addition to this, for fast recovery doctor can also inject alteplase
directly into the are a with clot by inserting a catheter within the artery in your groin and
threading it up to brain of patient suffering from thrombotic cerebrovascular accident.
5. Using a wellness approach to health, how will you ensure that Yun’s physiological needs
have been met?
It is important for care taker to use proper wellness approach to health for Yun.
Respective patient wants to go home because his physiological needs are not getting fulfilled
and he is found of food. But in hospitals it is not possible that psychological need of patient
gets fulfilled in appropriate manner related to food. Apart from this, other physiological
needs are safety and security so, as an care taker it is my responsibility to ensure that Yun
feel safe and secure in hospital when treatment is provided to him related to thrombotic
cerebrovascular accident (CVA). Along with this, me as care taker will help Yun in satisfying
their psychological needs by providing better health care services and ensuring his fast
recover in effective manner.
6. Using a wellness approach to health, how will you ensure that Yun’s psychosocial needs
have been met?
There are several psychological needs of patient such as safety, security as well as
many more which patients want to get fulfil. It is responsibility of care taker to ensure that
each and every need of patient will get fulfilled in appropriate manner. Moreover, food is also
part of psychological need but in hospitals it is not possible that psychological need of patient
gets fulfilled in appropriate manner related to food (Ungprasert and et. al., 2016). In relation
of Yun it is my responsibility to make sure that his each and every psychological need get
fulfil in appropriate manner as required by him.

7. Yun’s wife expresses concern to you about her ability to care for Yun at home. Describe
two (2) referrals you could make to assist her in Yun’s care. Identify and describe one (1)
support service available for her as a primary care giver.
Wife of Yun is her primary care taker and he ensure that each and every need get
fulfilled in effective manner (Hu and et. al., 2017). But I am medical professional according
to which it is my responsibility to give some guidance to Yun’s wife that how she has to take
care of him in effective manner. Thus, she has to make sure that Yun involve in daily
physical exercise because this will help in weight reduction which is important for Yun.
Along with this, it is also essential to take proper diet with less salt because high salt in food
result in high blood pressure which is harmful for Yun.
