Health care sector is very essential to make citizen of country healthy by solving their
problem in an appropriate manner. It is necessary for medical specialists to remain careful and
deal with patients in proper way so that complicated condition of person can be treated
successfully (Bergdahl and et. al., 2013). There are various cases in which severe complications
are observed then it is mandatory to closely monitor the patient and conduct treatment procedure
effectively as well as efficiently to sort out specific health problem. A single mistake or
negligence of employees of hospitals may lead to critical situation or death of the patient. This
report is based on given case study that includes crucial medical situation of patient named as
Ysabel Green. The assignment will discuss about clinical practice issues and health & safety
standards to care for a patient.
MAIN BODY
In the given case, various kinds of clinical practices are applied by medical specialists in
which certain issues are observed in care of Yasble. There are different types of medical cases
which are observed on regular basis in which some are complicated and require more care and
attention to diagnose them (Hinsliff-Smith, Gates & Leducq, 2012). It is necessary for medical
professionals to examine health situation of a patient very carefully and then apply appropriate
treatment accordingly. According to the given case, Ysabel Green a 33 years old lady having 3
daughters along with medical history of asthma, cholelithiasis pre birth of her 3rd child and
gestational diabetes. After delivery of third premature baby in 37the week, she had experienced
some episodes of right upper quadrant pain and nausea. Ysable was admitted to emergency
department of hospital A where she did not get any relief from medications and then her husband
admitted her to hospital B where they provide Morphine 2.5 mg IV 1-2 hourly prn and
Ondansetron 4 mg IV to relieve her from nausea & vomiting. Thus, they also conduct several
medical investigations that involves FBE, U&E, LFTs and an abdominal ultrasound (Shiu and et.
Al., 2012). There were several clinical practice issues which are observed in given case among
which some are given below:
Early discharge involves the situation of Ysabel in hospital A where she was initially
admitted but did not get any relief from medication and continue experiencing severe pain and
vomiting for 24 hours. It is necessary to monitor and examine several hormonal levels of patient
in an accurate manner to make sure that now the condition is stable and if any problem is found

than the discharge certificate should not be provided. In this case, previously the local health care
home where Ysable deliver a pre mature baby and therefore it was required to keep her under
observation to ensure that she is medically fit and in a condition to be discharged.

bile duct which get blocked in case of Ysable when gallbladder get burst at the time of surgery.
Negligence in care taking refers to ignoring behaviour of medical staff members when

of their superiors (Cope, 2014). It is essential for medical professional to closely monitor
individuals to analyse their actual conditions and take required action to make them relief from
particular health problem immediately. In case of Ysable, medications were given to her by staff
nurses but it was never rechecked that weather the treatment given to her is working or not. Her
condition therefore started becoming worse.
Postponed surgery repeatedly can be explained about postpone surgery can be explained
as to make patient prepare for surgical process and then delay that due to few of emergency cases
and fix on another day (Brinkert, 2011). Before conducting surgery, a person should not eat
anything before at least 12 hours and take light meal one day before operation. In case of Ysable,
she was already weak from some time and in order to prepare her for surgery she was provided

with light diet and was asked to avoid eating for long hours. This further made her condition
worse and therefore both Ysable and her husband Tom ended up with dissatisfaction.
Complications in surgery can be described as several critical issues which may raise
during the surgical procedure or created in post anaesthesia environment which results into an
emergency situation (Victor & Yang, 2012). It is necessary to examine proper medical situation
of patient so that surgical team will be prepared for possible complexities and sort out them
easily. In case of Ysable, doctors ignore her condition and consider other critical patients by post
ponding laparoscopic cholycystectomy. Thus, it will make her condition more worse, she
become orally nil and gall bladder get burst due to delayed surgery which blocked bile duct by
gall stones and then thy conduct MRI next morning to known about actual situation of Ysabel.
In case of Ysable, there are several national health and safety standards that have been
breached while providing care in care-home. Moreover, different kinds of health and safety
standards are very essential factors which are mandatory to be followed in hospitals and other
care homes while providing services to users (Forsberg and et. al., 2011). It is necessary to make
an effective as well as efficient criteria to carry out various kinds of treatment in an accurate
manner which helps to get desired results. There are different types of rules and regulations
which are made by government authorities to maintain health and safety level in country that
provide support to gain healthy future successfully. Some of national health and safety standards
that have been breached in given case are given below:
Instruction, Training & Supervision of Employees refers to concept of providing an
appropriate training and learning to staff member in terms of handling critical condition of
patient by giving suitable medication. It is essential to make them perfect because in medical
field everything should be done with perfection as a minor mistake may lead to death of patient.
In case of Ysable, medical professionals do negligence with her and avoid criticality of situation
by delaying surgery which results into burst of gall bladder that blocked bile duct and she will
remain orally nil as well as weak.
Preventing and managing pressure injuries includes different emergencies or serious
conditions that are faced in health care sector on regular basis which are required to be sort out
with more effective techniques (McMahon & Christopher, 2011). It is important to closely
monitor critical medical situation of an individual and conduct desired operation immediately

favourable to body response. In case of Ysabel, hospital B delay her surgery which leads to
severe complications including bursting of gall bladder and gall stones blocked common bile
duct. Hence, it is an emergency situation in which health care home should maintain contacts for
such conditions to provide an appropriate medication to stable position of person.
Preventing- falls & harm from falls can be described as to make more effective methods
or procedures and conduct them to protect people from critical situation (Wilson & Barton,
2012). It also involves to build an efficient processes or measure to prevent dangerous situation

repeatedly and giving light meal every night which make weaker which leads to make her orally
nil.

Medical safety includes the criteria of making an appropriate facilities for employees to

due to which she become weaker and orally nil.

maintain them in proper manner. It is necessary top build an emergency plan of action which
should be applicable in around all process or crucial situations. In health care sectors, many of

mandatory to create several critical condition units in hospitals which provide support to tackle
critical situations in desired manner to make patient feel comfortable. In case of Ysabel,
maintenance of health care standards in hospitals should be done by taking accurate actions and
few of them are explained further-

Induction training includes to carrying out more effective induction training in which

must be conveyed to employees. It is important to provide an accurate information to them so
that they will become capable to tackle serious condition of patient like Ysabel and use presence
of mind of control the same properly.
Strict regulations of employees refers to make strict and effective rules or regulations so
that staff members should conduct their every small or large tasks in proper manner (Dossey &
Keegan, 2012). Thus, it will render support to reduce critical situations in hospitals such as in
case of Ysabel and maintain level of safety which results into her healthy future successfully.

professionals in hospital A & B leads into critical condition. Several recommendations to make
changes in health care practices are explained further-
Effective learning and practical knowledge includes to make sure about effectiveness of
learning and practical knowledge because it has very important role in health care sector. It is
recommended that efficient educational programs which have meaning because it helps to
improve health care services to solve health issues of patient in correct manner (Kaşıkçı, 2011).
In case of Ysable, her medical specialists become capable to conduct more accurate diagnosis
carefully to sort out several health issues.
Government inspections refers to an activity which should be conducted by government
authorities to make sure about neglecting behaviour and other unethical or illegal aspects in
hospitals and stop them. Thus, it is suggest to carrying out appropriate inspections so that it will
provide support to prevent severe problems that are faced by patients as in case of Ysabel.
