This Student Assessment Booklet includes all your tasks for assessment of CHCDIS009
Facilitate ongoing skills development using a person-centred approach.
ABOUT YOUR ASSESSMENTS
This unit requires that you complete 4 assessment tasks. You are required to complete all
tasks to demonstrate competency in this unit.
Assessment Task About this task
Assessment Task 1: Written
questions
You must correctly answer all 16 questions to show
that you understand the knowledge required of this
unit.
Assessment Task 2: Case study This task requires you to read through a case study
and then answer the questions about identifying and
formulating development needs, strategies and
goals.
Assessment Task 3: Workplace
project
For this task, students will need to complete a skills
development plan for three clients with a disability
Assessment Task 4: Workplace
observation
There are two parts to this task.
Part A requires the student to implement a
person-centred strategy and skills
development for one client.
Part B requires the student to evaluate the
client’s skills development and provide
meaningful feedback based on client
response and observation.

Provide three examples for each of the following skills:
Life skills:
1. An example of life skills that help to manage or control the daily activities such as car
maintenance, First Aid, emergency Preparedness and other driving. These are basically
consider as important approach in regular routine of individual people.
2. Creative thinking among individual people- if someone can share their innovative idea to
the business manager where improving the overall business performance. Another
example is that when increasing market share so that implement or launch a new
product within specific marketing budget.
