Introduction
Communication errors have been prevalent over the years and have
a pivotal role to play in communication especially among the
healthcare settings.
Over the years communication has been considered as a multi
factorial along with a multidimensional phenomenon along with the
implementations of a complex, dynamic process which has been in
close relation to the environment and would include sharing of
individual experiences.
Effective communication has been considered as the key element in
the provision of high quality of nursing ultimately leading to patient
satisfaction and escalated health.
In settings pertaining to the healthcare effective communication
has been instrumental in reducing anxiety, pain, guilt and diseased
symptoms.
It has led to an increase in the acceptance, compliance along with
cooperation of the medical team along with proper functional as well
as physiological status of the system.
Clinical errors have been those errors in conduct or in judgment
especially in the clinical environment irrespective of whether not
there has been sufficient production of the error in the main cause or
in cases of an adverse event.

Incorrect language interpretation- This has been considered as a
clinical error in healthcare as it has been associated with
miscommunication and in the case of Mrs Druhi . It has been
caused as compared to videoconferencing along with person
interaction. Language interpretation skills lead to increased cases
of mental isolation as well as depression.
Risk in patient safety- Since Mrs druhi is an immigrant , she has
been facing communication problems mainly due to the lack of
cultural incompetence an lack of intercultural communication. Thus
it is causing impaired care and support to the patient. Moreover
there is an increased chance of breach of confidentiality as the
details of the personal information can be disclosed by the nurse
accounting to the lack of effective communication.
Cultural incompetence Cultural competence is defined as the
acknowledgement of an important part inclusive of the healthcare
systems which mainly focuses n the basic principle of culture . It
also includes the assessment of cross cultural communication.
Moreover the concept of cultural incompetence highlights the
vigilance in the dynamics resulting from cultural difference . It is
considered as an important clinical error as cultural incompetence
would often lead to the mental isolation of the patent. Moreover
there would increased miscommunication leading to the increased
dissatisfaction and mental agony regarding the proper provision of
care and support from the patient (Vermeyer et al. 2015).
Potential consequences of errors
Conclusion
Communication barriers constitute an important factor for effective
communication in healthcare.
Mrs. Druhi has been an immigrant from Nepal who has communication issues
due to linguistics barriers. The various clinical errors considered here are :
cultural incompetence, lack of intercultural communication, patient safety
compromisation.
Thus the effective formulation of a care plan ad the establishment of a strong
therapeutic relationship is required for escalated quality care in healthcare.

The main problem in the case of Mrs Druhi
Neupane who is an immigrant from Nepal and has
been admitted in the hospital due to the abdominal
pain. The identification and significance of the
clinical errors in healthcare is as follows:
Communication errors- Mrs Druhi has been facing
difficulties in the effective interaction along with
incorporation of the effective care plans and
interventions.
Cultural incompetence – It has been evident as Mrs
Druhi has not been able to communicate the
feelings properly due to lack of communication
mainly resulting from linguistic barriers.. Improper
interpretation of the nurse would hamper the
effective nurse patient relationship.
Risk of patient safety – Lack of understanding and
interaction between the healthcare provider and
the patient would lead to increased mis-
interpretation along with mis diagnosis of the
symptoms.
For effective communication with the nurse and an effective
patient nurse relationship, there should be an establishment of a
stable therapeutic relationship between the health care provider
and the patient. The nurse along with the other healthcare
providers should be flexible with the demands of the patient.
There should be effective intercultural communication along with
their patients preferably from varied cultural communication to
establish and ensure intercultural communication.
The nurse should use written instructions for the patient for
strengthening the communication and providing a care plan.
There should be a holistic patient centered approach focusing of n
the needs of the patient along with the implementation of a
patience approach.
Moreover the nurse should empathize with the patient for
addressing the care and needs and maintain the confidentiality of
the personal information of the patient (Kravasiliadou et al. 2014).
The safety issues for Mrs Druhi would be compromised due to lack
of understanding instruction and ineffective communication due to
leakage of information which is confidential
Mental isolation issues would be seen evident for Mrs Druhi along
with various health disparities and depression.
Lack of self confidence will be seen for Mrs. Druhi would escalate
the patient’s vulnerability.
The reduced quality of patients quality care.
