Inter-professional practice is termed as the collaborative practice of different healthcare
providers along with the family members of the patients to enhance the health outcome of the
patient. This discussion is also based on the Inter-professional practice to deliver quality care to
the chosen patient. The patient is a 67 years old aboriginal individual, who has been admitted to
emergency department for the recent fall and have a primary complaints of difficulty in
breathing, influenza A and increased confusion. The patient has a past medical history of arterial
fibrillation, chronic kidney disease. After assessing the patient, it is identified that the patient is
suffering from dementia. He lives alone after the death of his wife. The family of the patient is
involved in the care of the patient, and requires further help of the aged care and social worker
for the proper management of the patient.
The professionals chosen for the discussion to improve the physical and mental condition
of the patient is the social worker and the physiotherapist.
Social worker aims to improvise the wellbeing of the patient by the help of different
training, support and other resources. The social worker in a health organization is to help the
patient and the family of the patient about the process of disease from hospital to home and co-
ordinates the whole transition process. It actively participates in the patient discharge planning
also.
Another healthcare professional involved in the care of the patient is the physiotherapist.
Physiotherapist helps the patient to recover from injury or illness by the aid of different physical
activity, manual therapy, advice and education. In the healthcare setting, a physiotherapist aids in
managing pain of the patient and as well as prevents any further health deterioration among the
patient of all ages.
