The promotion of community health requires implementations of several interventions
and a wide range of factors that affect human health (Vollman, Anderson & McFarlane
2017). Population health services will ensure that people of all ages will be applicable to
receive an efficient health service on their needs. In the implementation of community health
nursing services, several educational concepts and theoretical frameworks are used to ensure
a good nursing care plan. To visualize the key features of this assignment, a concept map
(Appendix A) targeting the prevalence of diseases caused due to vitamin D deficiency in the
children of the Canadian community will be provided here. The following intervention will
work according to the processes stated through the nodes of the concept map.
Community identification:
A community health assessment begins with the identification of the target population
(Vollman, Anderson & McFarlane 2017). Nursing standards require the use of several
educational concepts to identify the risk (vitamin D deficiency in children) and promoting
appropriate and economically efficient care for the patients. The primary focus of the
proposed concept map will be to discuss the occurrence of vitamin D deficiency in children
and the health problems associated with it.
Community health and the role of nurses:
Social, economic and nursing management
The social determinant of any healthcare practice is the income factor. Vitamin D
deficiency in diseases involves the weakness of bones and many diseases associated with it.
The treatment of these problems needs the application of various drugs and nutrition changes.
As a result, the need for a cost-effective treatment strategy will ensure better care for the
children of the Canadian community (Vollman, Anderson & McFarlane 2017).

Nursing management requires the use of different educational standards and the
implementation of proper knowledge in the identification of the diseases associated with
vitamin D deficiency. Effective treatment plans can also be devised by the nurse, by
applying the educational standards (Vollman, Anderson & McFarlane 2017).
Social, primary, secondary and tertiary interventions
Social interventions will involve the use of several public awareness programs and public
education to aware of the Canadian community about the health problems related to the
deficiency of vitamin D in children. Primary and secondary interventions will both aim at
preventing the occurrence of the diseases associated with the deficiency of vitamin D in
children (Kaakinen et al., 2018). Tertiary interventions will involve the use of several
vaccines to cure the disease that occurred due to a deficiency in vitamin D.
Conclusion:
On a concluding note, it can be stated that the concept map provided in Appendix A is
justified by the explanation provided above. A better future can be hoped of, provided the
concept map associated to vitamin D deficiency in children is followed.
