2DATABASE DESIGN AND DEVELOPMENT
1) Name of the Project:
Development of Diseases and Patient Database in Microsoft Access.
2) Purpose of the Database:
The purpose of the database is to store the information of the diseases and patients. The
database can answer the questions of the business.
3) Description of Entities:
The project will focus on diseases and patient entities. Only these two entities has been
chosen for this project. Each of these entities have different purposes and different information.
As the entity name suggests, this entity will store the diseases that a patient have ever
encountered or even the diseases no has been diagnosed with. The disease entity will store
information like the name of the disease, its unique ID, primary treatment, possible tests, main
medicines, symptoms and more. The primary treatment will include a short description that how
to do the initial checkup until the test reports are not received. The possible tests can have more
than one test name separated by coma. The symptoms are like vomiting, headache, fever and
more. These are also separated by comas. The other entity, patient, will have a unique id, name,
residential address, contact number, email address, emergency contact number, contact person
name and more. As it can be seen, both the entities have a unique key. This unique value
illustrates that this value must be different for every row. This way, the system can easily
establish relationship among entities and also find data efficiently.
