INTRODUCTION
Heart disease and Osteoporosis are now more frequent in people with the process of
ageing. This is important for health and social care organization to provide better knowledge and
information to the people in order to change their lifestyle and encourage them to change their
habits to avoid these type of diseases later in life. Heart disease can be explained as a condition
that includes diseased vessels, structural problems and blood clots that can reduce the efficiency
of heart. As per the data of World Health organization, 17.9 million people die each year due to
cardiovascular disease. This number is 41 % of all deaths around world. This high frequency of
cardio vascular disease is major concern for health care organizations all around world. Some
heart diseases that are most common in current time are- coronary artery disease, heart failure,
Arrhythmia, valvar heart disease, aortic aneurysms and heart infection (Pérez-Molina and
Molina, 2018). These are most common heart disease that can cause severe condition or can lead
to death. To improve over all health condition of country this is important for health care
organizations to make people aware of these diseases and provide them information regarding
the causes of this heart diseases. This information could be very helpful for people to make
relevant changes in their routine work to avoid diseases related to heart.
Apart from heart diseases there are many people around world are suffering from
osteoporosis which can be explained as disease that occurs when human body loses much bone
or produce very little bone or both conditions at similar time. In this condition human bone
become much vulnerable and too weak. Osteoporosis also known as porouse bone and in this
condition bone loses its density that reduces the strength of bone in body. Worldwide there are
200 million women are suffering from Osteoporosis. It is more severe condition in old age where
it could be very difficult for people to manage their daily life with this condition. These two
diseases are major concern for health care organization around the world. There are different
health educational programs are used by the health care organization to make people aware about
these two diseases (Cooper and Marshall, 2018). The main objective of these educational
programs is to improve the knowledge of people to improve their lifestyle that can help them in
case of these two diseases. The educational programs can be considered as campaigns that are
used by the health care organization to provide detailed information to make people aware of
different health conditions and diseases. By this process government of the country will be able
to improve health condition of people throughout nation.

Background
Public health is one of major concern that government of country have. There are
different health programs are performed and plan by the government of the country to achieve
better health conditions in country to boost the growth and development of country. There are
some educational programs are also used by the health care organization to improve the
awareness in people to reduce the health cases around the world. Heart diseases and osteoporosis
are some diseases can be prevented by making certain changes in lifestyle of people and regular
habits of people (Carney and Freedland, 2017). This educational health care programs are based
on the process where different health officials of health and social care organization used to
provide specific information that is related to causes, reasons, risks and prevention methods of
these heart diseases and osteoporosis. This educational health care programs and campaigns are
used by the government to improve public health in country (Kearns, Schmidt and Glantz, 2016).
Educational health care programs are known as practices that are performed by the health care
organization to improve and promote health and fitness that usually offered them on different
places to suggest effective changes in their lifestyle to prevent them from different heart diseases
like heart disease and osteoporosis.
This main purpose of this project is to teach people about physical, mental, emotional,
and social health of people. It motivates and encourages the people to live a healthy and safe life
without any major health diseases. This educational programs provide critical information that
can help people to maintain their health standards and prevent different type of disease and
health issues.

The main objective of this project is to develop a plan for educational program that is
based on concept of providing information related to health care to prevent diseases related to
cardiovascular and osteoporosis. In this project detailed information of educational program will
be presented that can help to implement educational health care program efficiently. Various
aspects of educational will be discussed in project to make this educational program more
successful and cost efficient.

Justification
The number of patients suffering from heart disease and osteoporosis is been rising
significantly. In 2016, the estimated prevalence of CVDs in India was estimated to be 54.5
million. One in 4 deaths in India are now because of CVDs with ischemic heart disease and
stroke responsible for >80% of this burden. It has also been analysed that 50 million people
living in India are suffering from osteoporosis. Prevalence rate of this disease is higher in women
than men. 24.6% male are suffering from this problem while 42.5% females are going through it.
Most of the women are above 50 years of age. Thus, it is very essential to make people aware
and educate them about these diseases. Lifestyle factor of individuals also enhance prevalence of
these disease among people.
There is lack of education programme for managing and preventing exercise control for
heart disease and osteoporosis which is also one of the major reason for prevalence of these
disease. Thus it is really essential to develop an education programme which aims at
management practices. Project will be related to make patients aware about early signs and
symptoms of cardiovascular disease and reason for lower bone mass commonly known as
osteoporosis. This programme will assist service users in improvising their health outcome. It
has also been analysed that obesity is major factor because of which individuals are facing cardio
vascular problems.
Preventing disease is one of the most significant aspect so that there is an improvement in
management of service users so that long term risk and complications can be avoided. Health
care professionals needs to engaged in educating patients about the importance of exercise in
preventing heart disease. Treatment like medication and surgery can improve health outcome of
service users suffering from cardiovascular problems. High blood pressure and cholesterol are
major factors which rises risk of heart issues in patients.
Managing and preventing heart disease and osteoporosis requires keen monitoring by
health care professionals. They can also make use of person centred care in which care workers

Statement of intention
Main objective of this project is to educate individuals related to managing heart disease
and osteoporosis by preventing exercise control. Health care professionals needs to be engaged
in adopting various preventive measures related to heart disease and osteoporosis. Project also
intend to enhance quality of life of individuals who are suffering from cardio vascular problems
by laying emphasis on the management and leadership aspects of care workers, nurses and health
care professionals.
Background
Heart disease basically refers to that illness in which blood vessels got blocked or narrowed
leading to that condition where pain in chest increases. It leads to situation of heart attack. It
severely affects heart muscles of an individual. Osteoporosis is a condition where bones become
too weak making an individual unhealthy. Common signs which can be seen by an individual
suffering from osteoporosis includes back pain, sometimes even can cause fracture in bones. It
has been analysed that in India around 54.5 million individuals have been affected from heart
disease. This problem is significantly being rising day by day. These diseases are being life
threating like a small heart stroke can lead to many other severe problems like heart rupture,
heart failure. It has also been analysed that this problem also occurs when individual is obese or
engaged in consuming excessive liquor or smoking cigars.
In order to conduct this study population group which was taken was male and females
between the age of 35 and 55. Study has also laid emphasis on patients who has family history of
heart disease and osteoporosis. Thus, Education Programme for Exercise Prevention of Heart
Disease and Osteoporosis aims at improvising health conditions of people suffering by laying
emphasis on making them aware and educating patients about signs and symptoms of heart
disease and osteoporosis. Health care professionals will also have engaged in motivating services
users to engaged in doing exercise so that they can reduce risk of cardio vascular and heart
disease.

pain in chest, problems related to breathing, numbness, nausea, fatigue. Heart disease can also be
caused if service users has family history of suffering form problem of heart. In Early stages of
developing cardio vascular disease, service users may face issues like irregular heartbeats,
dizziness, easily getting tired. These symptoms when ignored can develop into severe life
threating problems. It has been analysed that heart diseases can be treated, if they are detected at
early stage. But usually patients are not aware and educated by early signs and symptoms. They
are also not aware about benefits of physical exercise which can assist them in preventing
problems like this.
In the opinion of Cooper and Marshall, (2018) Osteoporosis is a bone disease that can be
faced by an individual when the body loses too much bone. It makes bone too weak, because of

Budget

mass. Project will only be successful when knowledge of service users will be improved through
education programme conducted by clinical care setting (Al-Hayali and Mahdy, 2020). Success
can also be measured when service users are being engaged in doing different type of physical
exercise so that risk and vulnerabilities related to this project gets improvised.
Healthcare staff: Staff has suggested that this project will be beneficial to them when
there are reduced admission of patients who are at high risk of suffering from problems related to
heart and reduce bone mass. It will also support them in managing and coordinating tasks. They
can also assist patient in reducing their anxiety and depression level which has raised because pf
heart and bone disease.
Hospital administration: From the perspective of hospital administration, success can

Government/NGO/Health insurance/authority: issues concerning less knowledge of
patients related to heart disease and osteoporosis will be reduced if this project becomes
successful. It will also be successful if proper education is being given to service users about
benefits of exercise and early signs and symptoms of heart disease plus Osteoporosis. From the
government point of view success will be measured when health outcome of service user has
been improved by implementing and adopting education programme preventive exercise.
REFLECTION
I have analysed that leadership strategies have significant impact on success of educational
programme. Appropriate leadership approaches were needed to implement this programme. It
has helped me in achieving my goals and objectives. During my tenure I have learned that it is
really necessary to have an idea about circumstances of patients who are attending education
programme, so that changes in programme can be made according to them. This has assisted me
in providing proper education related to benefits of exercise to service users. I have also engaged
in making patients aware about signs and symptoms of heart disease and osteoporosis. This has
assisted me in improvising health outcome of patients.
As per the projects' outcome, I have strongly analysed that leaders play an important role in
dealing the management on health cares systems in term of hearts diseases and osteoporosis
education exercise trainings. There should have effective selection of leadership styles which
have appropriate for catering different strategies (Al-Hayali and Mahdy, 2020). In order to be
services more level of coefficients in sustainable and appropriate manner. In order to became an
effective leader, I have developed clear understanding in terms of management skills to cope up
with existing level of different situation. I have analogized that there is contentious amount of
change in health deliverables in order to have communications in more effective and flexible
manner. I established a great level of understanding according to different needs and objectives
of heats and osteoporosis management. The continuous learning and education programs can be
valuable in accomplishing this goal. However, the health professional leader is considered toe
have respectability in order to have identification of various barriers to motivates the teams

RECOMMENDATION
After going from the conclusion of the education programme that has bene used in Project
report, it has been analysed that large scale study related to cost effectiveness of education
programme related preventive exercise related to heart disease and osteoporosis can be
conducted. It has bene evaluated that providing service users with knowledge related to early
sign and symptom of heart disease and osteoporosis. This will assist them in improvising health
outcome of service users. It will also reduce mortality rate which is been rising because of this
disease. Another individual who will be going to make this project, I would recommend them to
be involved in doing prioritisation of task first so that no confusion arises. It is recommended
that report must be finished by addressing to all ethical aspects. I will also suggest not start the
report if individual does not possess specific knowledge related to signs and symptoms of heart
disease and osteoporosis. It is also being recommended that in order to make this project
successful multi-disciplinary team can also be involved so that they provide education related to
managing signs and symptoms of severe and critical diseases like cardio vascular and
osteoporosis. It is also being suggested that resources which are been used in education
programme has effectively used. This will help clinical care setting in making this program huge
success.
CONCLUSION
From the above report, it can analyse that Heart disease and Osteoporosis is considered to be
condition includes diseased vessels, structural problems and blood clots that can reduce the
efficiency of heart. These are most common heart disease that can cause severe condition or can
lead to death (Laroche and et.al., 2017). The objective of the report is to successfully have
organization of educationist campaign in by the health care organization to provide detailed
information to make people aware of different health conditions and diseases. These educations
training programmes have proper level of impacts over promoting health and fitness which is
being usually different places to suggest effective changes in their lifestyle to prevent them from
different heart diseases like heart disease and osteoporosis. In this report, there is identification
of clear justification over patient awareness along with managing and preventing exercise control

these disease. It has been analysed that physical exercises help in better flow of blood reducing
