Question 1:
The case study represents the case of Mr Gold, who is an elderly man suffering with
many co-morbid conditions. The health care issues that the patient had experienced in the
past include right above the knee amputation, diabetes with ketoacidosis, coronary artery
bypass graft, along with vascular problems and neuropathy associated with his diabetes.
Although, the presenting complaint of the patient is the osteomyelitis, for which he required 6
weeks of intravenous antibiotics. Along with that, another notable concern for Mr Gold is the
fact that he had not been working for the past few years and is surviving solely on the social
security deposit and his wife’s part time job. Hence, along with the impact of the health care
concerns that the patient is suffering with, the impact of the financial strain is also a huge
impact on the health and wellbeing of the patient and his presenting condition. It is very
important for his nursing care delivery to be very interactive and educative, so that the patient
can understand the exact life style changes that he will require to implement in order to
manage all of his disorders and health adversities successfully (Alligood, 2017).
There are many theoretical models in place that help in the characterisation and
development of adequate nursing care planning and implementation of the care planning
(Smith & Parker, 2015). One of the models that can be implemented in the care scenario of
Mr Gold is the conservation model by Myra Estrin Levine. This model is based on the
ideation “nursing is a human interaction”, and the theoretical model deals entirely with the
interaction of the patients with the nurses, emphasizing on multiple factorial interactions.
This theoretical model helps in producing predictable effects using probability as the reality,
which is why this model has been chosen for this case.
Question 2:
The conservation model by Myra Estrin Levine focuses entirely on the fundamental

concept of Nightingale’s nursing ideologies, “the nurse created an environment in which
healing could occur”. In this case, the care for Mr Gold will not just be focussed on
improving the physical health status, but will also be focussed on improving his overall
health. There are four principles of the theory, the principle or conservation of energy,
principle of the conservation of personal integrity, principle of the conservation of structural
integrity, and principle of the conservation of social integrity (De Chesnay & Anderson,
2019). Hence, the care planning for the patient will also involve conserving all the mentioned
components. Although, the case study provides a vast array of data, both subjective and
objective, on the patient, understanding the structural integrity, social integrity, personal
integrity and energy, the nurse will require to explore and understand the patient on a more
personal level. Hence, the nurse will have to communicate with Mr Gold, and explore his
beliefs, thoughts, outlooks and lifestyle preferences including his present care issues in acute
detail.
Question 3:
The conservation model of nursing is intricately associated with a more holistic
ideation of healing, involving physical, psychological, social, emotional and spiritual health
and wellbeing (McEwen & Wills, 2017). Hence, the care initiation for Mr Gold would be
based on proper exploration of the present conditions of the patient, understanding what he
perceives as health and wellbeing. The care for MR gold would be very culturally safe,
holistic and patient centred, empowering the real choice and uniqueness of the patient, his life
and his condition. Similarly, the care initiation will be based on development of a therapeutic
interaction between the patient and the nurse, Ms Frank. Moreover, this theoretical model
focuses on the holistic care planning and incorporates empowering the patient choices.
Hence, in that case as well, Ms Frank would need to interact with the patient and provide him
holistic education regarding the available options for treating all of his presenting concerns,

and help him take the care decisions for his disease management, preferably with the
assistance from his wife.
Question 4:
Different nurses have a different work approach, and their care techniques also vary,
depending on a variety of factors, including personal nature, resilience, and educational
qualification of the nurses (Cherry & Jacob, 2016). Similarly, the theoretical models used by
the nurses also has a huge impact on the kind of care planning and implementation carried out
by the nurses (Booth & Kaylor, 2018). In this case, the conservation model had been chosen
on the basis of applicability and critical analytical thinking, although the in case Susan had
chosen any other nursing model, undoubtedly the impact on the care planning would be
entirely different. Elaborating more, whereas the conservation model is based on the
interaction and environment, other models such as, the Inter-system model by Barbara M.
Artinian focuses on intrasystem interaction development within the human system. Similarly
the Conceptual system and theory of goal attainment and transactional process model by
Imogen M. King focuses on transactional benefits associated with goal attainment in care
planning (McEwen & Wills, 2017). In both examples, the focus of the care planning is
diverted from the holistic care or nurse-patient interactions, which is the integral element in
this case. Hence, taking the assistance of any other model would implement a drastic
difference in the care planning.
