INTRODUCTION Nursing is the practice of providing care for the vulnerable and those who are in need of care. Identification of health concerns is necessary to assess prevention strategies regarding health concerns(Rodrigues and et.al., 2018). In the given case, a male patient is having some health concerns. The project will analyse the actual and potential health outcomes and teaching plan for the patient in case study. Actual or Potential Health concerns In the given case study, the weight of the patient was 158 lbs which is normal for the males of this age. His temperature and oxygen rates were also assessed as normal. However, form his check-up following concerns were reported: Actual health concern In the given case study, the actual Health concerns for the patient is High blood pressure because the patient is having blood pressure more than than the normal rate. As perFuchs, and Whelton (2020), the normal BP is less than 120 and over 80. Blood Pressure of 140 and above 90 is considered high blood pressure stage 2. there are various causes which can lead to high bloodpressuresuchasoverweight,obese,lackofphysicalactivity,excessivealcohol consumption, stress, genetic, family history and old age. It can also be caused due to sleep apnea. In the case study, the patient is an African descent who is above the age of 60. The major health concern for the patient is High Blood pressure as he is experiencing high blood pressure and sleep apnea. According to a study African-Americans, people above the age of 35 and people who experience sleep apnea are more likely to develop high blood pressure(American Heart Association, 2017). It is very important to assess the health concern of high blood pressure as it can lead to stroke, heart failure and atherosclerosis in patient. Potential Health concern The Potential Health concerns in the given case study can be heart disease as the patient is feeling dizzy, experiencing heavy episodes of severer headaches, high blood pressure and is unable to sleep at night. As per the views ofBenjamin and et.al., (2018), heart disease can cause hypertension, enhance Blood Pressure, stroke, heart failure and heart attacks. People who are over the age of 50 and have high blood pressure are more particular to get this disease. The patients experiencing
heart diseases can often develop re-current awakeness, which are also known as insomnia and sleep apnea. The patient works for almost 12-13 hours every day in a weeks and also looks after his parents. He is having high blood pressure, but is also experiencing dizziness and severe headaches. Dizziness can lead to potential fall risk. He is unable to sleep at night. These symptoms report potential health concern of heart disease in patient. His high blood pressure can also be a potential cause of heart disease.It is very important to assess the potential health concern of heart disease as it can lead to high blood pressure, difficulty in breathing and hypertension in patient(Del Re and et.al., 2019). Teaching plan Health promotion or illness prevention strategies are the programs undertaken to improve health at every stage of life. Strategies for actual health concern For the patient in the case study, regarding his actual health concern of high blood pressure following health promotion and illness prevention strategies can be used: Physical exercise:The patient in the case study works in a high tech company for almost 13 hours per day a week. He has a family of which he also have to look after. This limits his physical activities. Lack of physical activity is also causing sleep disorders and dizziness to the patient. Lack of physical activities can cause various heart diseases such as high blood pressure. Regular physical activities can lower the risk of high blood pressure. It can help in controlling the weight and reducing the conditions which might cause high blood pressure in the patient (Arango and et.al., 2018). Any physical movements such as taking stairs, gardening or brisk walking can help the patient to receive various health benefits. Healthy diet:In the case study, the lifestyle of the patient is very stressful and busy which might cause poor diet. Although the patient is having normal weight as per his age, but poor diet can enhance his actual concern of high blood pressure. A healthy diet can help in protection from various diseases such as high blood pressure. Healthy diet can improved blood pressure rates if a eating plan involves vegetables, fruits, whole grains and low fat dairy foods etc. Healthy diet also helps in better quality sleep.

Strategies for potential health concern For the patient in the case study, regarding his actual health concern of high blood pressure following health promotion and illness prevention strategies can be used: Stress management:The patient's lifestyle is very stressful and busy. This enhances the risks of heart diseases and unhealthy diet plans. He have less time to sleep and physical activities will contribute a lot in creation of stressful life(D'Alessandro andet.al., 2017). To manage stress various alternatives can be adopted such asphysical activities, yoga, exercises and meditation. These strategies can help the patients in improving their health and which ultimately leads to better heart health. Regular health screening:The patient was feeling dizzy and had experienced the episodes of headaches. After these incidents he went for check-up. He did not had regular health check-ups. High blood pressure and other causes can probably lead to damaging heart and blood vessels causing heart diseases(Pascoe and et.al., 2020). The regular screening of the blood pressures, cholesterol levels and type-2 diabetes screening can help in prevention of heart diseases. Regular health check-up can identify the symptoms of diseases at early stages and early identification can help in prevention of those diseases. CONCLUSION From the above discussion it can be concluded that, recognition and assessment of various health concerns is significant in order to prepare prevention strategies. In the given case, actual and potential health risks of high blood pressure and heart disease were recognized for which a teaching plan consisting of Health promotion or illness prevention strategies is provided.
