Identification of the intervention strategy to integrate health behaviour change:
Cardiovascular disorders attribute to a large proportion of the complex morbidity and
mortality cases within the Kansas City of the United States. Statistical evidence revealed that
the approximate death rate on account of cardiovascular disorders within the Kansas City is
equivalent to 156.5 deaths per 10,000 individuals (Heart Disease Facts & Statistics, 2019).
The primary risk factors that attributed to cardiovascular disorders within the population base
include unhealthy lifestyle factors such as obesity and increased smoking (Heart Disease
Facts & Statistics, 2019). The trans-theoretical model was chosen as the health change model
to integrate lifestyle changes within the population base so as to acquire positive healthcare
outcome. Research studies suggest that integration of health literacy can help to empower
patients in order to make positive lifestyle changes that can improve the quality of physical
health of the population base of the Kansas City (Velardo et al., 2015). Therefore, the chosen
intervention strategy for the project would comprise of imparting health literacy to affected
and vulnerable patients by means of a health promotion campaign.
Project Rationale:
The rationale for the choice of the intervention strategy can be mentioned as the
strong evidence base that assures the high rate of success of the chosen intervention to
promote a positive lifestyle behavioural change. A number of research studies have suggested
that imparting health literacy is the single most effective intervention to foster a positive
health behaviour change among the targeted vulnerable audience (Guzys et al., 2015; Velardo
et al., 2015).

Project Goal:
The project goal would comprise of disseminating awareness among the targeted
population to integrate positive lifestyle behaviours so as to reduce the heightened risk of
acquiring a cardiovascular disorder.
Project Objectives:
The project objectives can be broadly defined under the following:
To disseminate awareness in relation to the risk factors that trigger cardiovascular
disorders
To assist participants with effective weight management plan to prevent obesity
To assist participants with cessation of smoking
To encourage patients to lead an active and healthy lifestyle
Description of setting where the intervention would take place:
The health promotion program would be conducted within the clinical setting of XYZ
hospital. Patients who had been admitted to the hospital on account of cardiovascular
disorders would be enrolled into the program post their discharge for a period of 8 weeks so
as to integrate positive lifestyle changes and reduce the probability of recurrent
hospitalization.
Associated Rationale:
The associated rationale for the choice of the project venue and the participants can be
mentioned as the increased relevance with the project objectives. Further, the primary goal of
the project is to integrate a positive health behaviour among the individuals who are either
already suffering from cardiovascular disorders or are most likely to suffer from a
cardiovascular disorder. Further, the chosen intervention is most relevant and applicable to
