This vision document discusses the problems in the current healthcare system and proposes a patient record system to improve efficiency and patient care. Vision Document
1. Problem
NewAccess is the mental health intervention which targets the depression and anxiety
of people. In the current system, young people with the mental illness are seeing various
professionals before they are getting help they require. Each time they are visiting the
professionals, they require telling their story. Each time the patient clam up and they say
little than before story. Therefore, it is difficult for the professionals to help them in this
case. Following are the problems which are identified in the current business operations
of NewAccess such as:
I. When the patient re-tells their story, then it takes lot of times and the process
becomes long. Due to long duration, the start time of the treatment is also
delayed. The situation becomes worse.
II. As each time, the young adult tells the story to the professionals, and then the
details become little. The professionals get little details of their mental
problems.
III. If one of the patients provides little details of their problems, then wrong
treatment is started to them.
In this case, the system analyst proposed such a system which helps the organization to
capture the story first time it is being told by the patient to the professionals. The proposed
system is patient record system.

2. Capabilities
The proposed system provides integrated capabilities to the clinicians and healthcare
professionals as it helps them in the treatment Following are the capabilities of the
proposed system such as:
a) The system handles to record the clinical functions and patient’s story. It
records the medical information. The recording system can identify and
manage the patient’s records at the first time they visited the first
professionals.
b) The system can manage the problem lists and medication lists.
c) It can also manage the clinical documents and notes such that it helps the next
professionals to easily understand the case history of the patients (Lindblad et
al., 2017).
d) The system can record the story in audio form or written form as per the
organization’s functions. The audio will then transfer to the word so that it will
be helpful. All the story records should be stored in central repository with the
patient’s data.
e) Next time, when the patient will visit the next professionals, then the
physicians should update the new records which will review in this case.
Update of the new records will help next professionals to become familiar
with the changes if any in the treatment.
f) In this case, when the information or story of patient is shared with others,
then it is mandatory to take approval from the patients (Feo&Kitson,, 2016).
Without their approval, it is not possible to share the confidential and personal
data of the patients with other professionals. At the time of appointment, the
information is shared with the consultants so that they can understand the

entire problems that the patient is facing in this case (Ocloo& Matthews,
2016).
g) The system has capability to capture the external clinical documents so that
the treatment should be going properly and efficiently.
h) In this case, the patient’s feedback is also required so that it will be helpful for
the treatment. There is also required to update the feedback of the patient in
the story which help the doctor to understand the present situation of them.
Based on the present situation, the doctor can take possible actions and steps
for curing the patients (Hossain, 2016).
i) The system can also manage guidelines and patient specific care plans so that
it helps the organization to maintain a reputation in the market.
3. Benefits
Following are the benefits of proposed system such as:
1. The benefit of the proposed patient record system is that it can save time as well as
money of both patients and doctors. With using the recording and audio files, before
the patient visits the doctor, they should be already updated with their case history
about the mental problems that they are facing.
2. The system can manage the results and historical tests results to proper clinical
personnel for the review (Nardo et al., 2016).
3. The system should provide accurate and up-to-date information about the patients at
point of their care.
4. It enables to provide quicker access to the patient’s records for coordination with
them. It helps to provide an effective mental care services to the patients.
5. It shares of electronic information with patients so that there is no possibility to errors
in the story and patient’s records (Zhang et al., 2017).

6. It also enables the patients to provide safe and reliable prescribing.
7. It improves patient’s interaction with the doctors and improves into the
communication methods.
8. It also enhances privacy as well as security of the patient’s data so that there is no
possibility of sharing the data with third party (Feo&Kitson, 2016). All the data are
kept safe such that no third party person can access to get the data and share with
others.
9. It also helps to improve productivity of the healthcare organization.
10. It reduces the cost of the paperwork and reduces duplication of the testing.
