MAIN BODY
Overview of the healthcare system in UK
The United Kingdom has a universal healthcare system that is known as National
Healthcare system(NHS). The institution was founded in 1946 and was enacted in 1948. The

services , equity and central funding .All the provisions of the NHS is accordance with the UK
health care system. In the ancient time national insurance was introduces and under this the
system is provided with the medical coverage to employed person through payroll
deductions(Brenman, 2021). It has been observed that NHS in one of the largest public health
system in the world and serves on an average of 1 million people and have efficient standard
care. In addition to this, the institution has to follow various safety regulations and laws in order
to maintain peace and also provide medical facilities to every person of the country.
It has ben observed that in 1990 the UK government put more focus on improvement of
the and made on of the major changes in the NHS by creating the internal market. These markets
were independent organisation that were managed by the and competed with each
other(Greenberg, and et.al., 2020) . Furthermore, in 1998 UK government introduced the NHS
plan which assist in the development of the NHS. But in the present time the healthcare system
was operating with the provisions of the 1998 guidelines but on the other hand it lacks various
characteristics such as national standards, it lacked incentives and various means to improve the
performance(Maniatopoulos, and et.al., 2020). It has been observed that the health care system of
UK has undergone major reforms over a period. The NHS has undergone changes like changes
in the management structure of the government and financial model of the organisation.
In addition to this Brexit also has immense impact on the health care system of UK.
From the data collected it was observed that more than 5,000 nurses from the U countries left
the NHS from 2017-2019 but the current situation predicts that there has been increase in the
number of nurses that are coming from outside the EU to work in UK. The other major
challenges faced by NHS was the shortage of the medicines although , the organisation has the
stock of the medicines but it was all used during the coronavirus(Thorlby, and Arora, 2018).
Various members of Pharmaceutical company suggested that maintaining the stock of the
medicines can be an issue during the post transition period but importing the medicines from
other countries is the major issue that is faced by the country in 2021.
Theory of modernity and impact on the welfare state
Furthermore, the health insurance care system is governed and guided by NHS with the
purpose of publicly funding the various health care companies . From the research it was
predicted that government funding covers 85% of the healthcare expenditure in UK. According
to the modernity theory refers to the process of modernisation within the societies and states that
the traditional societies. Furthermore, it is the process of evolution of the society that results into
development of the community as well(Domingues, 2020). It also states the development of the
society and its economic growth. The current healthcare system in UK has undergone various
changes in the past decades. It is a government sponsored universal health care system that was
called National health services and consist of publicly funded healthcare system in UK(Strunz,

and Bartkowski, 2018). It includes all the health related services in England, NHS Scotland,
NHS Wales and health and social acre in Northern Ireland. Under the healthcare system of UK ,
people have the right to buy the private heath insurance. The NHS plan introduced and promised
more power and facilities to the patients and shorter appointments , improved healthcare for the
old age patients. Although it is public funded healthcare centre but it also has its private
healthcare sector in which the health care is acquired by the private health insurance(Habibov,
Cheung, and Auchynnikava, 2018). In the ancient times it was observed that many of the
patients were not gives effective treatment and not many facilities were provided i.e.
discrimination was done and high quality services were only for upper class people which led to
increase in the death rates. But with the evolution of the NHS it has focused on providing
effective medical services and treatment to the patients and various other medical facilities for
their safety. There has been various issues that is faced by the NHS post Brexit but it has
managed to overcome and provide the best treatment and other facilities to the patients(Taylor-
Gooby, and Leruth, 2018). It has been also observed that with the advancement in the technology
has helped to install machines that is required in the treating the patients and other various
benefits for the health and safety of the patients.
The welfare state of UK is considered as the safety net which is introduced with the
motive provide support the various sectors of the community and it includes the education ,
housing and health care. The healthcare sector is one of the major aspects of the welfare states of
UK for which NHS is established and provide medical services through a three-way structure of
primary care, hospitals and community health care. Healthcare is the second major aspect of the
welfare state in UK. The healthcare services provided by the welfare states of UK is through
NHS and are financed by taxes. The services provided covers the entire population and virtually
free for the patients. The organisation lays more emphasis on the patients who are need and need
medical help(Le Grand, and Robinson, 2018). The major issue faced by the welfare state
regarding the healthcare sector is the ageing population and need for medical assistance and
focused towards providing services to the patients and person in need for the medical treatment.
Contemporary citizenship
The contemporary citizenship is based on the national citizenship that is associated with
the ethonationalism and the other is social citizenship or it can be said as welfare citizenship that
refers to the social rights and also it is closely connected to the civil society institutions rather
NHS Wales and health and social acre in Northern Ireland. Under the healthcare system of UK ,

The contemporary citizenship is based on the national citizenship that is associated with
the ethonationalism and the other is social citizenship or it can be said as welfare citizenship that
refers to the social rights and also it is closely connected to the civil society institutions rather
the state or market(De Noronha, 2019) . There are four types of citizenship and the first is
related to the national citizenship which is connected with the concept of ethno-nationalism that
centred towards nation building. This theory was closely associated with the improvement of the
state and also became essential in the authoritarian states that followed the Korean war. The
main purpose id the development of the nation as well as for the society and make various
changes in the healthcare system in UK in order to provide better medical services to its patients
by introducing health insurance schemes for the people in need for the treatment. Furthermore, it
also focuses on providing free medical assistance to the poor people. The second includes the
social citizenship that is related to the civil society institutions rather than state or market. It also
includes the social rights that assist in the development of the welfare state and can also be
referred to as welfare citizenship. It is necessary to educate the patients and convey about their
rights so that they can get free treatment and not have to pay for the treatment given to
them(Fukuyama, 2018). The third form of citizenship is concerned with and identifies the
citizen with the engagement of the workforce, focusing on self- reliance and independence.
The last model of the citizenship presupposes a consumer society where the citizen
become the consumer of privatized goods and services and when these products are bought
online the consumer need not go to the stores. It has been observed that there are various issues
identified which creates barrier in the development of the community and its is important to
evaluates the problem and find possible solutions to the problem(Faulks, 2022). The major
problem is that in many areas people are not getting medical treatments which can badly affect
their health and it is necessary to establish various health care institution by the government so
that effective and free medical services should provide to the patients. Health is considered as
major factor which also contributes n the development of the society.
With the evolution of various factors in the environment has led the healthcare
institutions to adopt various new technologies and machines in order to provide quick medical
treatment to the patients which was not possible in the ancient times. But with the help of
government funding it has become possible to operate the healthcare system effectively and
provide medical services to the patients in need and free of cost(Tuli, and et.al., 2020). People
have to face various social issues related top health. These are related to the social determinants
which refers to the conditions in which they live , learn and work that affects their health. These
issues arise as a result of distribution of money, resources and power across the nationals. The

various social issues are-Class and income and disability(MartÃnez-Caro, and et.al., 2018). In
order to overcome these issues it is required to introduce new and effective health policy which
focus on reducing theses social issues and provide services equally to all the patients and special
provisions for the patients with disability as they require special care and attention.
Critical evaluation of theories of risk, vulnerability and resilience
Risk is considered as an important aspect in the modern society and the risk theory is an
attempt to explain the decision that is taken by the people when they face risk. Healthcare
organisation involves various types of risk which can be clinical , regulatory , privacy(Taylor-
Gooby, and et.al., 2020). It is necessary of the healthcare organisation to undertake and use
various tools and techniques that helps in identification of the risk and its outcome and evaluate
that helps in planning the appropriate risk management plans in order to reduce the risks that is
involved which can impact the organisation and its working. With the help of this theory it will
help in making effective decisions in order to avoid the risk involved(Lopes, and Jaspal, 2020).
On the other hand vulnerability is a complex that is applied on a large scale in order to identify
the person and groups. In the context of healthcare it related to the ethical determinism that
recognize the need that is to be defined in measurable and operational terms.
It has been observed that many of the patients are highly dependent upon the technology
care and other patients opt for therapies that results into cure but can have severe consequences.
Furthermore, it also lays emphasis that theory of vulnerability does not play a major role in the
vulnerability(Macrae, 2019). It also states that vulnerability creates stress and anxiety among the
person which also affects their physiological and social functioning and it states the person in
theirs situation can develop more health problems.
Understanding of the disciplinary functions of the welfare state as an agent of social surveillance
The welfare state refers to the concept of government that plays a very major role in
promoting as well as promotion and also economic and social well-being of the citizens of the
country. The welfare state follows the principle of equality of opportunity and equal distribution
of wealth. The government acts as an agent and keeps a control and watch over the social and
economic security of the country. The various types of function of the welfare state are :
ï‚· Regulatory functions: The welfare state has the responsibility for maintaining law and
order in the country and also ensure that organisation operating in any industry complied
with the laws and regulations applicable to that industry. They also have the authority to
order to overcome these issues it is required to introduce new and effective health policy which
focus on reducing theses social issues and provide services equally to all the patients and special
provisions for the patients with disability as they require special care and attention.
