INTRODUCTION
In the health and social care organization the senior health care professionals with the
huge experience need to lead and guide the new health care staff and workers for the
accomplishment of standard performance and cure of the patients. Effective leadership skills of
the health care professionals promote continuous development of the knowledge, ability, skills
for the improvement of the patient care quality, social health services betterment, safety,
compassion and the patient experience. The nursing senior is supervising the people with the
dementia care and treatment (Hawkins, 2021). Dementia is the health condition, this is the
impaired ability to remember, think or make decision that interferes with the every day activities.
The dementia care plan development and support team play very important role in the
management of the effective care of the service suers by facilitating and managing the key
services (Stickdorn, 2018). The key facilities of the dementia care are home care, adult day care,
adult family homes, continuing care recruitment, nursing home facilities etc., and for the
management of this leader need to recruit the skilled staff and guide them for the effective
performance. In this report key roles and responsibilities of the leadership in the leading teams
are going to evaluate in context to the Hannah new plan and team management for implementing
and developing new facility to support 30 people with dementia in a specialist dementia suite,
and 49 extra care apartments for those in need of extra care. Importance of the theoretical models
of the team building along with the stages of the team development and leadership style is
going to analyse in context to the achievement of successful project outcomes. Key
recommendation are going to illustrate in context to the team member and support team
development.

Leadership is play very important role in the health and social care task effective success
because this involve the clear guidance for the task, support and motivation for the work role,
leader play important role in the management of work load stress and solution of the health
social care challenges,. Below is the description of some key roles and importance of the
leadership in the health and social care team development and leading.

Recognize the skills and capabilities- In context to the Hannah new project they goal is
to design and planning of a new facility to support 30 people with dementia. Hannah need to
recruit the new staff for the different job role such as carers, administration and reception,
maintenance, domestic and catering etc. in context to the accomplishment of that task effectively
she has to focus on the key role of the leadership, which is the identification and recognition of
the employees skills and abilities according to which they can further make the dementia health
care job role differentiation among the newly recruited staff. Leader role is building the team
and in the team building leader must recognize the skill and capability of all team member's and
empower them to work with their full potential (Haslam, 2020).
Leaders create positive work cultures- Another key responsibility of the leadership while
leading the team is managing the positive work culture. Such as Hannah should build the
positive culture with their expectation for the action and behaviour. This culture must high
quality, safe and the responsive use of the resources. Leader should exhibits the honesty and
integrity in all activities, leader should serve as the role model for the team member's to ensure
that they will act in a similar manner. That is the leader role to eliminate the intimidating
behaviour for the successful management of all these roles Hannah should establish policies
that encourage, facilitate and reward the desired changes in attitudes and behavioural throughout
the organization.
Foster innovation and change- Hannah have to recruit the new staff and in that task their
is need to implement some initiative and changed recruitment strategies for the recruitment of
the employees with the selective, specific and innovative skills of the health and social care
services. This is the key characteristic or role of the grate leader to make difference in the place
of employment for fostering innovation and change. Hannah should work to improve the
outcomes and patient experience by re-think and redesigning healthcare and developing
innovative models of care. Hannah can create the new health care model with the improved
strategies and interventions and social challenges solving guidelines for the subordinates work
efficiency improvement.
Envision a desired future- Another important role which should be manage or accomplish
by the Hannah is having the vision for the appropriate working environment. With the exiting
staff Hannah should espouse a powerful vision with which subordinates get agree and want to
be the part of the vision and highly motivated to act. In context to the achievement of any vision

destination of the vision. That is the responsibility of the leader to communicate effectively with
the employees about the organisation mission and vision in order to help employees to make the
sense of there work should connect with organisation goal. The clear vision gives the team
direction and helps people to focus on the understanding that how to participate their job well

In context to the development of the effective team in health and social care leader must
use the appropriate theoretical model's as it is valuable for the successful team development in a
systematic manner. Hannah should make the analysis of the different theoretical models of the

2021).

team .

goals, strategy and ground roles. In this stage, team leader need to provide the strong direction
and supportive environment which can help to develop confidence within team members. This
can be effective to accomplish the team goal together. To avoid any risk of chaos within mean,
autocratic leadership style can be more effective. This can help to associate team leader with
their leader and leader direct the key action which should be performed by the team member.
This can be effective and allow to provide better management of team at the initial stage. This
autocratic leadership can be effective where leader take all the decision with little input of team
members. through this team is able to perform well and help to achieve the successful project
outcomes.
Storming- This is the second stage of team development which is critical and difficult to
go through. This is the stage in which there can be occurrence of confidence that can also include
the competition of individual with other team member. This is the stage in which team
productivity may get decrees which is due to increased competition and conflict. In this, team
member can get disagree with team goals and area of agreement. In this, leader have the specific
role to direct every team member which can be effective and allow to get better outcome form
team. This can help to get better managing team through resolving the team conflict. There is
need of team leader in order to take decision because due to conflict within team member there
can be risk to take right decision within team. There is also need to provide the information
about every member role that can allow to work according to their role to accomplish the task. In

member to unlock their potential and to identify their skill and values which can be effective for

member of team support each other and help in performing their role. This is the productive
stage in which team work effectively with each other. In this, there is no need of leader to guide
the team. Every member of team work effectively and help each other when there is any type of
conflict and resolve it more effectively. This is the stage which can lead to provide better
positive outcomes and team member have their role to achieve the goal of the team development.
In this type of team development stage delegation leadership is effective. In this type of
leadership, there is no need for guidance for the group members. In this, leader should not
delivery their decision to team members. In this stage team get capable of making decision
which is effective and allows to get better understanding about the team development. There is
no need for the motivation in this stage, low level of motivation can be more effective and allow
to get better interaction with team member for the productive outcomes (Brown, and Pehrson,
2019).
Performing- This the fourth stage which is more effective and team are at their peak in

with team member with asking their opinion. In this, leader ask every member of team about
their decision which can be helpful to get better understanding about the number benefits. It
allows for creativity which promotes team to get spirit along with corporation from their
employment. This allow to perform their role with different style to complete within time. This
can be effective and allow to leader to believe on their team which can be effective for providing

Adjourning- This is the last stage of the team development in which team goal has been
achieved and it is the time to adjourn the team. This can have said as the termination of the team
which can be effective and allow team member to say goodbye. This is the last stage which

stage team has accomplished the task. Projects final task and documentation has been completed.

which can be done easily with the open communicating.

which can be more effective and can allow to provide better and effective communication to the
team.
It is recommended that team leader should involve the team member in the decision
making where with the help of open communication, team member are able to put their views
with the team leader which can help to take better decision. This can be effective and allow team
member to get better relationship with other team member which is effective and have better
outcomes. Open communication is the key element which is more effective while making any
decision. This allows to put views of every team member which allows to get better and effective
respond from the team member for performing better output.
It is also recommended that team leader should ensure that every team member are giving their
best while performing task and should get support when needed. This can be achieved when
there is open communication within every individual of team. When team member are free form

most important theoretical model is model of six characteristics development, this describe the
key steps and aspects of the health care team development, such recognition, membership,
support for change, maturity, participation, problem solving. The five team development stages
which should be followed by the team leader are forming, storming, norming, performing, and
adjourning. There are the different types of the leadership styles which must be consider by the
leaders such as participative, autocratic etc. One of the most important recommendation for the
communication between team members to support team development is listening to the
employees and task them fro their input. Another recommendation are having an open-door
