The information that should be shared between family members an care givers are:
All the past medical conditions, complications and surgeries should be conveyed to
the care givers so that they could understand the complexity of patient conditions.
The patient should be provided with medical attention and the family members should
work along with the care giver so that they could develop effective care process.
All the critical health concern, both physical and mental that the patient is suffering
from or has the risk of development should be shared among the patients so that
they could be prepared and could help in the effective treatment of the patient. (Surr
et al., 2017)
Besides these, all the risks related to medication errors, elder abuse, effect of
neglect and family concerns should be shared within the family member and
caregivers so that effective treatment could be developed and implemented in the
care process (Badr, Carmack & Diefenbach, 2015).
While caring for older adults in their homes, care professionals and family members should be
able to implement the preventive measures so that the elderly patient could be protected from
adverse health conditions (Selman et al., 2018). The tips for their preventive treatment are:
1. Their common senior healthcare concerns such as falls, incontinence, dementia,
pain, depression and isolation should be addressed effectively.
2. Family members should be able to optimize the critical healthcare conditions.
3. Their emotional wellbeing should be addressed by determining their emotional
wellbeing (De Palma et al., 2018).
4. Their physical health should be promoted by including them back in physical
activities.
5. Healthcare facilities should always be communicated for any critical health
condition (Peimani et al., 2016).
