Abstract
This assignment is about the description of milestone comes in a path of an individual while
launching the health awareness program to get rid of chronic illness. This assignment will
emphasize on financial and evaluation plans. This program requires human and financial
resource in which an individual has to arrange funds from sources like donations and crowd-
funding to recruit medical professionals. A benchmarking tool used to analyze the effectiveness
of a program and patient’s data is measured to ensure the proper working of this plan.

Resources utilize in the program
Chronic illness health awareness program develops by an individual in the pike county to
aware people to make them more serious about their health. For executing this program, it is
essential to identify the resources that will utilize in the program is mention as below:
Human resources- For launching health awareness program, the staffing requirements identifies
by program manager to meet the needs of recruiting professional medical and Para-medical staff.
This will consist of certified doctors, nurses, pharmacists, physicians, volunteers and social care
homeowners (De Mauro, Greco, Grimaldi & Ritala, 2018, pp. 807-817).
Financial resources- Funds will arrange from the state government and raising donations from
the community users of the pike country to launch this campaign effectively. Crowd funding
source of finance is also to use by an individual to raise the funds (Marín-Vinuesa, Scarpellini,
Portillo-Tarragona & Moneva, 2018).
Impact of the program on the organization’s current budget
Current program is consider as wide program whose motive is to aware all the audiences
and the community members of the pike county to know the significance of their own health.

This program will not fit in the existing budget of the organization as this will involves enough
fund in arranging the venue, purchasing medical equipment’s such as wheelchairs, arranging
ambulances, catering (Seymour, 2018). Recruitment of medical professionals and volunteers for
this event requires heavy sum. The social workers will also help in this campaign in enhancing
people’s spirit in taking initiative for their own and someone else’s health and keep their nation
illness free.
Suggestions for ensuring the financial sustainability of the program
It is advisable to the organization to search for different sources of arranging funds to be
financially stable for launching this program on the floor. Sources of funds which an entity will
need for meeting the project goals and the objectives will consist of crowd funding, a subsidy
from the government, and donations from the Kentucky health department (Hissa and et al.,
2019, pp. 23-39). The program will successfully implement by meeting the funding needs of this
project. It is recommended to use the dashboard system to keep track of all the activities of this
event to track its progress that all are completed on time or not.
EVALUATION
Evaluate the effectiveness of the program
Checking patient’s health history- In checking the effectiveness of a program, the previous
health history of patient is analyze such as disease of the patient, Blood pressure, allergies of
patients (Snyder & Deters, 2019, pp. 219-224).
Nutritional and diet plans- Pre and post diet plans is given to sick person which will measure
by identifying the nutritional values of all the foods included in the diet of an individual.

Health benchmarks- The benchmarks will help in measuring the effectiveness of a program that
the package will able to achieve the end targets or not.
Technology in treatments- Measuring the data of the technology utilize in treating the diseases
using robotic surgery, virtual reality, laser in surgeries.
Patients who will attend the event- The information of all the patients who attended this self-
management program to fight against the chronic illness.
Tools to measure the effectiveness of the program in reducing the health disparities
Patient safety- The safety of the patient is ensured by giving entry and exit to a patient in the
campaign only by checking codes given to them (Bright, Callahan & Saunders, 2019).
Clinical process- Electronic clinical quality measures utilize by an organization in which patient
card is given to a patient to recognize a person’s health history easily by scanning their health
cards.
Patient and Family involvement- Questionnaires is to be filled by the health executives in
seeking the views and opinions of all the patients and their family members about the health
services offer to them in the health camp.
Checking the program will get successful or not
The progress report of all the sections of this health awareness program is compared with
each other to know its effect. The final result of all of these is matched with the industry’s
benchmarks to ensure the successful implementation of this program. Taking feedback of the
patients attending this event will also help in ensuring the success or failure of this program. The
weakness of this program is considered for a future event.

To what extent the program will ensure healthcare equity across its populations
This program will open for all classes of the society in taking benefit of all the health
services offers during the event. Patients are categorized into different color codes to allow a
doctor in recognizing one patient from another. This will, in turn, avoid isolating a diseased
person by an individual. Counseling is given to all the patients to spread healthcare equality
among the community members of the pike county.
