INTRODUCTION
Obesity, hypertension and diabetes type 2 may termed to be chronic issue which is most
common disease nowadays. These disease may generated through age, eating habitat, laziness
and so on. People majorly suffer with number of issues may termed to be harmful for their health
and can lead to death. To prevent these disease there can be organised intervention which should
be consider in order to control and prevent these disease.. in this following report, there is
discussion of case in which an individual is dealing with these disease. The report will cover all
essential aspects that may provide a better understanding in order to undermine the concepts of
such chronic an complex diseases (Arshad & Gan, 2019). .

Mathew is a 33 year old Aborginal Gentleman, who have history of obesity, dibetes type
2 and hypertension. As he seeking treatment in an out patient clinic at Farmdale Community
health centre with the local dietitian suggest that he can observe with help of local dietitian. The
local dietitian. As the consultant suggest that he can local GP to get assistant through for medical
treatment for weight loss management. As he is aware that there is link with in obesity, diabetes
type 2 and hypertension.
Identify all aspects of patient care relevant to Mathew’s chronic conditions
In the case of Mathew, as in the child his wight is normal as he diagnosed through
multiple diseases which are diabetes type 2, hypertension, and obesity, he tend to have put on the
weight. The role of practitioner termed to be significant as it can promote the prevention and
improvement Of such chronic diseases. Traditional way pf improvement can be concluded
through number of authors which it may demonstrate change of dietary plans, elimination of
food which contains sugary or may contain large amount sugar element in it such as chocolate,
nutrella and so on. The role of health practitioners can be termed to be vital as they can provide
quality of care and assistance to each individual in order to prevent and control their living
standards. Nurse involvement practitioner's role starts with monitoring and detection of blood
pressure and individual's education may effect and has expanded to be one of the most effective
term strategies to improve BP as well as blood sugar level in a body makes a major difference as
