Context of the Patient
Mrs. Carolina Morris, a 46-year-old female suffering from ulcerative colitis
since her teenage years. With her condition worsening she is undertaking
ileostomy surgery, as per her doctor’s suggestions. She is discharged on
26th March 2017 to go home with a visit from a “stoma nurse”. Mrs. Morris
works but unable to work in the past year due to her condition. PMH:
ulcerative colitis x30y, diarrhea for the past year, weight loss up to 20kg.
NIL Known Allergies. The current educational plan includes ileostomy
medication care that she can take at home.
Cues collected from the scenario
Mrs. Morris has Pain 8/10 with a minimum incidence of nausea as
reported. “NP insitu at 2L”. “N/S at 100ml/h”. Her ileostomy bag has a
clear brown output of 250 ml. The dressing over stoma has been clean
and dry. However, the patient refuses further pain medication and is
sipping water and not drinking due to increased output in the bag. She
has been given Ketorolac 10mg QID PO as the medication. Paracetamol
was not given by other nurses. She has literate English magazines by her
bed and has family support. She feels anxious and scared about having an
ileostomy and appears uncomfortable laying on her side on edge of bed,
not ambulated yet.
