1. Provide the child with drawing materials. Make sure they are child-safe
2. Instruct the child to draw any picture he would like to draw
3. Once completed, have the child explain to you his drawing
4. Jot down your observations throughout the activity. You may use the guide questions

5. Once done, scan the child’s work to save in his records

He was drawing the picture of monsters

Yes, this shows that he was having some mental issue like anxiety which is distracting his
psychology and he is expressing it by the mean of drawing monsters

Describe the child’s attitude while completing the task:
Child was active while completing the task, however he was noticed to be confused and sense
of nervousness was also analysed.

Was the child able to understand the instructions without difficulty? Yes | No
If not, explain the nature of his difficulty:
No, Samuel is a quick learner as he quickly grasp the instruction given regarding the drawing
activity. He responded positively and displayed his skill of art with full efficacy. The only
thing noted in Samuel that he was needs his parents around him to work with sincerity.

Provide an example of a story the child shared with you that helped your assessment of the
child’s general disposition:
On investigation with Samuel about the concern he showed to be around his parents, revealed
the reason for drawing monsters. He narrated that he fear to be around strangers as once he has
been into the situation where he was tried to be harm by an unknown person. He draw monster
to portray stranger peoples.
5. Provide an anecdotal information you gathered from the child’s parent that is consistent with
your own observation:
On communicating with Samuel’s parents about his anxiety and drawing monsters, their parent
also confirmed that his child always needs someone whom he can trust. He became stressed
and anxious when he meet with any stranger person as he assumed that they will also try to
ham him. Sometime, he has been into situation where he start to yell at night out of fear. They
said that, they have tried to keep Samuel in stress free environment and that is the reason is
