Introduction
This report is based on the public inquiries into the failure of health systems for the
delivery of safe and evidence based care. In recent years, it has been observed that there is an
increment in clinical errors, clinical incidents, and patient deaths. Therefore, this report focuses
on two modules of public inquiry such as clinical governance and organizational culture.
Clinical governance
Clinical governance is defined as the system wherein the governing body, managers,
clinicians and staff members shares the accountability and responsibility for quality care,
continuous improvement, minimization of risks, and creating and environment of quality in care
for customer or patients.
An effective system of clinical governance at all levels of the health system is essential for
the insurance of continuous improvement in the safety and quality of care. Good clinical
governance creates certain that there is accountability and creation of culture which embraces
report and support for the improvement. As per clinical governance, patients are centered for the
identification of safety and quality difficulties and the resolutions which requires to be
implemented. There is a rehabilitated and supported importance on management, philosophy and
development as being important to high quality upkeep. The outline classifies the schemes which
are obligatory for the growth and upkeep of high presentation group (Victorian Government,
2008). The schemes are prepared into five provinces and reinforced by incessant monitor and
improvements:
