Person centred care demand in increasing day by day as it is very helpful in providing
right treatment to patient. This type of care services lead to focus on crucial aspects of care such
as full filling the need and demand of patient as well as fast recovery. It is very important for
care giver to have required skill set and ability to meet with the requirements of patient. This
report will includes different rules and legislations that is related to health care sector. In this
report different types of person centred care are determined as well as goals and objectives are
developed in order to improve the capability of person cantered care provider. Apart from this
person development plan is created in order to increase skills of personal and professional
growth.

defined manner (Bing-Jonsson and Et. AL 2018). With the assistance of basic requirements with
the help of which government and public to ensure person-centered practices get conducted in a
well defined and lawful way. Further this has been evaluate that, with the help of effective legal
support organisation can avoid any legal consequences and obligations. Mentioned below there
are some certain form of legislation that are being followed by NHS in context with person-
centered care:
Health and Social Care Act, 2008 (Regulated Activities) Regulations 2014-

of Mr. X, I have to be more encouraging and motivating. I have to be more supportive to Mr
X as he needs to do lots of physical exercise and follow proper diet plan in order to recovery

It is very important for an individual to analyse and evaluate different skill set that is
required to perform task in better manner. There are various types of potential skills and

my performance for conducting task and activities in effective manner I have used SWOT
analysis. This is an effective tool that will help in determining individual strengths, weakness
as well as opportunity and threat. By using this method I will be able identify what are my
core skills and abilities as well as analyse those area in which I need to improve. This will
also help in analysing what are various opportunities that can be accomplished by developing
skills and knowledge.
Personal development plan is very supportive in increasing skills and capabilities of a
person. In order to develop ability and potential as a personal centred professional short,
medium and long term goals are mention below.

interval for check up These check up are very important for fast recovery of patient. In recent
time I did not communicate to my team member about check up calling of dementia patient.

of dementia patient. I have done necessary checkups and provided proper medicines. I have
also developed a proper plan for dementia patient which includes regular check up for

lifestyle is the major issues of Obesity problem. Dementia is mental health problem where as
