collected from human participants are required to gain ethical approval before commencing their
research. Please answer all relevant questions and note that your form may be returned if

Researcher will inform participants with the help of email in order to make request to
fill survey. In addition to this, it will be assured by me that respondents will opt out
any time as per their convenience. Participants will be guaranteed with classification
and security of data provided by them.
How will you obtain consent from participants? Will this be written? How will it be made
clear to participants that they may withdraw consent to participate at any time?
Verbal as well as written consent will be taken from respondents. Along with this they
be allowed to opt out accordingly to their wish.
