INTRODUCTION
Healthcare professionals are highly effective to delivery better care and support where
nursing staff plays an important role in delivering better care and support to patient. In this, there
is need work collaboratively which can allow to provide better and effective response towards
delivery of better and effective healthcare services to patient. In this, there is also need for
multidisciplinary team where different healthcare professional along with different agencies
work together to delivery better care services to client for their better health improvement and
facilitating care services. Here, nursing role is highly important which can monitor and provide
different support within healthcare premises for better health improvement of patient health
(Qadan and et. al., 2020).

QUESTION 1 :- Discuss the role of the multidisciplinary team in the effective discharge home of
a 90 year old man who is living with dementia and was admitted to hospital having burned
his arm in an accident in the kitchen.
Multidisciplinary team refers to the team of different healthcare professional who are having
speciality in different disciplines like social worker, nursing staff, general practitioner. They
together provide their service which can help client to improve their health. Here,
multidisciplinary team includes different healthcare professional in the case where 90 years old
man with dementia has presented in hospital due to getting burned arm which occur in the
kitchen as accident (Erfani and et. al., 2019).
There is different role of multidisciplinary team which includes prioritising client health
condition which can help in taking care of client current health and ensure about delivery of
effective care and treatment that can help in improving client health. They are eligible to provide
integrated care to client which can allow to ensure about the positive outcomes which are
important for them. They should use their speciality and provide better care and service that can
include delivery of better treatment and care to the client. Here, client is represented due to
getting burn on arm due to kitchen accident. Here, patient has dementia and due to having old
age client need better care and support which can enable multidisciplinary team to discharge
patient from hospital to home.
