This document provides a template for assessing individual learning goals in nursing practice. Individual Learning Goal 1

Perform clear and effectively during handover to nurses as well a other health care professionals
when need be using using appropriate medical terminology by the end of week 2 of placement
Rationalise why this learning goal is important for your practice as a nurse and how
it relates to one NMBA RN Standard for Practice and any sub-standards.
Communication is the passing of message to one another (Johnson and et. al., 2018). Nurses are
the backbone of the healthcare system meaning that a large responsibility of care falls on the
nurse, so it is imperative to have clear and effective communication . Effective communication is
essential in the health care system such as, prevention, treatment, therapy, rehabilitation,
education and health promotion (Odberg And et. al., 2018).
Nursing is very essential as nurses assist patients and their relatives who cope with variety of
illnesses, and the way to deal with it (Harrison and et. al., 2020). The learning is important as
when nurse have to take care of patients, man responsibilities have to be fulfilled including
handover. This goal effectively utilises the NMBA RN standard and its sub-standard of
“maintaining the capability for practise” (Cleary and Hungerford, 2020). That shows how to
perform the activities while handing over to other nurses or the professionals. As per the standard
the work that is performed by professionals or nurses should abide by the maximum capability in
the practise. The goal also addresses the aspect that responsibility while handing over to another
professional is one of the key aspects that need to be considered. This aim assist to adhere
according to NMBA RA standard related to “capability”. Usually these activities may cause
different kinds of error if the relevant information is not effectively shared by the RN. Yet if the
information are shared accurately with the handover of practise, then there will be no error in
serving the patient. Therefore, the following standard help the nurse and professionals to transfer
the duties and information in a manner that nursing practise will be helpful.
What are the specific steps you will take to achieve your learning goal
There are a variety of steps that I will effectively conform with in order to achieve the set
objective:
• To achieve the goal I have to regularly learn from my senior at the workplace. I
will analyze the working style and time management they follow. The seniors will
help me to understand the process and provide knowledge related to handover of
responsibility and practise so that I can reach to the goal more effectively.
• For achieving the goal I will make use of I-PASS mnemonics as it covers all the
aspect that will help me to analyse the severity of the illness so that I can explain
that to the person to whom the practise is to be handed off. The information
related to patient that is under treatment, the activities that have to be performed
while handover the work. With this I can increase awareness related to the
situation and synthesise the receiver. This ultimately assists to achieve the targeted
goal.
• To achieve the goal I also make use of EMR which consists of a well developed
and systematic collection of patient information in digital format. It will help to

transfer the information related to the patient to other nurses. Therefore, I will
easily share the summarised information of patient to the handover nurse.
• Experiencing the situation repeatedly is another step that I will take so that I can
be more efficient in the practise of hand off as I have to learn from the various
situation that occurs in the process of handing off.

Rationalise why this learning goal is important for my practice as a nurse and how it
relates to one NMBA RN Standard for Practice and any sub-standards.
This goal is important for my practice because as nurse the knowledge of medication
administration assists the patient and their families to take less stress (Young and et. al.,
2017). With this knowledge, all of them will be more comfortable when they have
knowledge about the distributed medication and correct use of them (Parand and et. al.,
2018). Moreover the goal to get confidence in medication administration is crucial in my
nursing practise because it will assist me to suggest specific medicine to patient. As per
the standard of NMBA RN the goal have similar aspect of sub-standard that is
“evaluating the outcome to inform the nursing practise” (Härkänen and et. al., 2020).
As per the goal and the sub-standard the evaluation of outcome of medicine will
contribute me in understanding the benefits as well as adverse effect of medicine. The
goal is directly linked with the standard of NMBA as I can understand the outcome of
prescribing the medicine to particular patient for its better health and well-being.
According to this sub-standard the outcome of medicine is highly valued in this because if
the patient will be prescribed right medicine and right dose, then this will help me to get
medication administration (Thomas, Donohue-Porter and Fishbein, 2017).
What are the specific steps you will take to achieve your learning goal
Now, in order to to achieve the learning goal that has been developed, there are a variety
of steps that I plan to take:
• To achieve the goal I will learn about the medicine that help the patient to improve
their health. Therefore, medicinal knowledge will assist me to understand
medication administration.
• I will closely monitor the intake and delivery of medicines and the times so that it
can effectively help me in understanding how different doses are to be
administered and how these can impact the patients.
• To reach the target I will regularly check the labels of medicine so that right
medicine is prescribed to right patient.
• The amount of dose to be given to the right patient will also learned by me so that
the goal can be achieved in proper standard.
• The working environment is also necessary to assess the medication
administration as when I will work in positive environment then I will understand
the use of medication more effectively.
• I can also improve my medication by keeping regular updates in this field of
medicines and the quantity to use and the particular reason to use the medicine.
Communication with the patient is also necessary to provide prescription of
medicine to them and I will make sure that patient will not face any side effects of

the medicine.
• I can confidently manage the Medication administration through study and by
working under an experienced person so that I will get to know the condition as
well as disease's cause and precautions related to it.
