WHAT IS DIABETES?
PURPOSE OF THE LEARNING
PLAN
It enables the community members to become
more knowledgeable about the disease and
self-management within a multidisciplinary and
collaborative setting.
Diabetes occurs when the blood sugar level in the
body becomes too high. Insulin, a hormone made
by pancreas help the glucose from the food one
eats, get into his or her body cells for being used
for energy.

DIAGNOSIS OF DIABETES
Anyone with a body mass index higher than 25,
regardless of age, who has additional risk
factors, abnormal cholesterol levels, sedentary
lifestyle, and having a close relative with
diabetes.
Anyone older than age 45 is advised to receive
an initial blood sugar screening
Any woman who has had gestational diabetes,
is advised to be screened for diabetes every
three years.
Anyone who has been diagnosed with
prediabetes is advised to be tested every year.
Following are the screening guidelines by ADA
(American Diabetes Association (2015):
