Title
The title of this poster is “Chronic heart failure due to Ischaemic Cardiomyopathy and
hypertension”.
Clear introduction including outline for the poster
This poster presentation demonstrates the brief description of topic and its relationship to
education, practice, research and policy. It also critically appraises evidence-based studies
related to the topic. This presentation examines the methodological aspects of each. It also
summarises the articles and the main findings. This poster also addresses the limitations of
the review.
In addition, usual care group could be considered for obtaining resources related to self-care.
Along with this, proceedings phone calls were used in 30 days by subsequent discharge with
taking into consideration of feasible treatment. Primary result would be readmission of
hospital as well as, secondary results would be related to quality of life (QOL), utilisation of
healthcare, self-care, as well as, events related to major vascular (Triposkiadis, et. al., 2016).
It is found that there are 6 wards in hospital that were randomised within two groups for
study purposes. As per the allocation of ward, total of 140 respondents were participated and
these were fall under teaching back and usual care self management intervention for heart
failure.
On the other hand, scores of self-care enhanced in both groups within 1 month was highly
enhanced in intervention group as well as, in the context of control group, confidence and
maintenance scales was increasing. However, there is lack of sub-scale related to self-care
management. There is also lack of difference were identified in emergency department
presentations and quality of life between groups. There are high amount of patients within
control group as compared to intervention group who have visited within health care services
